About this role

Job Description

  • Monitor drug therapy for assigned patients, assessing therapeutic endpoints and potential adverse effects by evaluating relevant lab and diagnostic parameters. Contacting prescribers with patient specific recommendations to improve outcomes, decrease adverse effects and/or lower costs. Accepting prescriber changes in therapy and communicating these changes per procedure with the patient’s nurse.
  • Consistently, evaluate and intervene when appropriate after being alerted by the surveillance software rules which include  renal  and hepatic dosing , anticoagulation monitoring, antibiotic stewardship or other specific alerts. Provide pharmaceutical services for  outpatient &  inpatient pharmacies. 
  • Review and reconcile admission orders, allergy and medication histories clarifying any discrepancies with the prescriber, patient and family. Reconcile any new orders post transition of care with pre-transfer orders clarifying any discrepancies with the prescriber.
  • Actively participates in patient care rounds and treatment team  as requested, focusing on interventions and education efforts to optimize patient drug therapies and improve medication prescribing patterns.
  • Provide drug information services to staff and patient education as requested.
  • Identify Performance Improvement and Quality Assurance initiatives for the Pharmacy Department. Complete daily pharmacy Quality indicators ,  Participate in the design, data collection, and presentation of QA and PI initiatives proactively monitoring core measure patient’s drug therapy to assure compliance and intervening when appropriate with the prescriber.
  • Assure compliance with National Patient Safety Goals and Medication Management standards of The Joint Commission.
  • Document all clinical interventions utilizing the surveillance application, reporting any investigation of adverse drug reactions and medication errors. Communicate with health care providers any recommendations/modifications of therapy when appropriate.
  • Complete pharmacy quality indication on daily basis. 
  • Perform handoff communication ensuring appropriate transition of patients to incoming and outgoing shifts.
  • Manage storing , dispensing , labeling, distributing and recordkeeping of Investigational Drug.  
  • Participate on Investigational Drug training as required per protocol. 
  • Participate in Outpatient  & inpatient Clinical trials conducted at the hospital.  
  • Participate on pharmacy on call services and cover weekends / evenings  if business require. 
  • Supervise and direct pharmacy support personnel verifying daily activities of the pharmacy technicians as well as participating in their appraisal.
  • Supporting the management, oversight and operation of all aspects within the inpatient &  outpatient  pharmacies which  includes : 
    • Ensure Pharmacy Professional Practice compliance with State and Federal regulations 
    • Patient Safety & CQI program 
    • Conduct Drug utilization Review

    • Participates in Pharmacy Diversion Control program
    • Participates in Pharmacy Loss Prevention program
    • Pharmacy Workflow Management
    • OTP program Recordkeeping 
  • Manage pharmacy  inventory ordered from the  wholesaler and other suppliers to  ensure Outpatient & Inpatient pharmacies will have adequate supplies 
  • Manage Automated Dispensing cabinets ( Pyxis ) and Pyxis quality assurance program. 
  • Other duties as assigned by director
