We are seeking an empathetic and driven Senior Social Worker to join our Urgent Care Acute and Prisons Service in Morecambe Bay on a full time, permanent basis

Key Responsibilities

  • Provide a single point of access to community intermediate care services, including reablement and residential rehab, 7 days a week.
  • Support hospital discharges and work with the wider multi-disciplinary team to prevent hospital admissions.
  • Utilize the Discharge to Assess model for assessments away from the hospital environment.
  • Conduct holistic, face-to-face Care Act Assessments, reviews, and support planning using a strengths-based approach.

Requirements:

  • Registration with Social Work England (SWE) or application to register with SWE.
  • The post is covered by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (as amended in 2013). Successful candidates will need to apply to the Disclosure and Barring Service (DBS) for a 'disclosure'.
  • A car for use in connection with the duties of this post, insured for business use. Consideration may be given if you cannot drive due to a disability.
