Position Purpose:

All KHI employees are expected to perform their respective tasks and duties in such a way that supports KHI’s vision to be the kindest, highest quality health systems in the country.

Under the general supervision of the Clinical Director, and clinical leadership of the Registered Nurse (RN), and in collaboration with the clinical team, the CNA performs clinical nursing procedures and functions within the Arizona (AZ) CNA Scope of Practice. The incumbent supports the plan of care for each patient, as determined by the health care team, and in accordance with the goals of the organization. Works to improve patient safety and patient experience.

 

 

 

 

Key Responsibilities 

Certified Nursing Assistant:

  • Responsible for attending to the daily living and safety needs of an assigned group of patients..
  • Assist registered nursing personnel in the care of patients by performing various patient care activities and related non-professional services.
  • Collaborates and coordinates with other departments to provide timely effective care consistent with individual’s needs, choices and preferences.
  • Obtains and records vital signs, heights and weights per policy.
  • Meets patient’s personal hygiene and basic needs in a timely manner.
  • Ensures that the patient is comfortable, necessary supplies are available and in reach.
  • Keeps patient environment safe and tidy.
  • Provides patient specific care as appropriate to age of patient.
  • Demonstrates effective communication skills with children, adolescents, adults and geriatric patients to include use of appropriate therapies such as play, written, and picture board. 
  • Reports changes in patient’s condition, out of normal values, patient/family concerns or complaints to nurse, charge nurse and/or supervisor.
  • Applies AIDET as well as other evidence-based behaviors. 
  • Participates in developmental and organizational initiatives for process improvement and quality assurance as requested.
  • Follows appropriate infection control procedures.
  • Performs skin care interventions and treatments, as permitted by state regulation. 
  • Maintains patient and staff confidentiality.
  • Assists in the orientation of new staff as requested.

 

Unit Specific Responsibilities:

Med/Surg, Peds, PCU, ICU, Observation Unit, and ER:

  • Under the direction of a registered nurse (RN), provide continuous visual and proximal contact with the patient(s), including but not limited to in-room observation, provision of basic comfort services, maintaining a safe environment, accompanying patient during transportation for tests/procedures, and documentation on appropriate forms
  • Maintains educational requirements and training requirements to fulfill the role and responsibilities of a Unit Coordinator as outlined in the Unit Coordinator job description.
  • Regularly updates patient communication boards.
  • Performs hourly rounds with patients.
  • Performs bedside shift report.
  • Assures patient belongings are accounted for on admission and transfers and returned at discharge. 
  • Performs designated admitting tasks.
  • Records accurate intake and output for each patient.
  • CNA may be required to provide backup support a Camera Tech in specific units:
    1. Demonstrates competence with operating video monitor equipment by managing the video monitor (including audio component), display of rooms, adding and deleting beds monitored; is aware of actions for video monitor Downtime. 
    2. Receives report on video patient assignment from nurse. Stays at the monitor at all times, reports changes in patient’s behavior/status to nurse. 
    3. Documents and signs “Record and Video Monitoring form” (verbal reminders, staff interventions, # of fall alerts. 
    4. Identifies the behaviors that may precipitate a fall; is aware of the proper steps to take if patient is not seen on the video monitor.

Home Health and Hospice:

  • All documentation is completed accurately and timely (no later than 8am the next day).
  • Productivity to average no less than 5-6 patients per day.

Hospice House:

  • Performs hourly rounds with patients.
  • Performs bedside shift report.
  • Assures patient belongings are accounted for on admission and transfers and returned at discharge. 
  • Performs designated admitting tasks.
  • Records accurate intake and output for each patient.
  • Performs designated admitting tasks.

 

Qualifications 

  • Must be 16 years of age or greater, if CNA
  • Must be 18 years of age or greater, if LNA

Education: High school diploma or equivalent, if providing POC Testing

Experience: Previous experience as a Nursing Assistant, OR successful completion of a Nursing Assistant certification program

Licensure: N/A

Certification: 

  • Current and valid Certified Nursing Assistant Certification, if CNA 
  • Current BLS through the American Heart Association or American Red Cross

Unit Specific:

Pediatrics:

  • Two-week pediatric orientation with annual competency in pediatrics

Home Health:

  • Level one fingerprint clearance card, or application, required within 20 days of hire.
  • Current and valid Arizona Driver’s License, must be 21 years of age or older and clean driving record.

Hospice and Hospice House:

  • Level one fingerprint clearance card, or application, required within 20 days of hire.
  • Current and valid Arizona Driver’s License, must be 21 years of age or older and clean driving record.
  • Current and valid Arizona Food Handlers Card.

 

Preferences 

  • Experience as an LNA or CNA in related unit’s field
  • High School Diploma or GED equivalent

 

Special Position Requirements 

Exposure Categories: Category I: Expected duties have routine potential for exposure to blood, body fluids or tissues

Unit Specific:

Home Health/Hospice 

  • Requires travel, in properly insured personal vehicle, throughout the service area.

 

Work Requirements 

  • Must be able to distinguish appropriate colors, if providing POC testing
  • Able to handle multiple priorities simultaneously
  • Demonstrates good oral and written communication skills
  • Able to assist with the movement of an adult patient weighing up to 300 lbs.
  • Able to stand, bend, stoop, kneel, and reach, while working with patients for the entire shift
  • Able to care for patients with infectious diseases
  • Ability to sit for 3-5 hours per day
  • Able to stand for 3-5 hours per day
  • Ability to walk for 3-5 hours day
  • Able to care for patients in a variety of settings
  • Proficiently utilize office equipment including phone, computer, fax printers, copier, scanner, etc.
