A liaison to the medical nursing unit who provides communication between all disciplines as well as patients, families, and visitors. Health Unit Coordinators are responsible for accurate order entry and management of patient medical chart.

Education

JOB RESPONSIBILITIES

Essential

Greets visitors and interdisciplinary team members with a smile and provides assistance as appropriate within the scope of their training.

Collaborates and communicates with the nursing staff to maintain accurate records during the patient's hospitalization. Communicates effectively with different departments and people. Assists staff with communication within and between departments.

Keeps charge nurse informed or admissions, discharges and transfers by frequent communication. Performs electronic updates to appropriate systems for admissions, discharges and transfers (i.e. patient disposition, assignment management application, throughput application).  Accurately enters patient data into computer on all admissions, discharges, and transfers. Accurately and legibly maintains staffing assignment sheets.

Utilizes appropriate resources for clarification of orders. Transcribes orders as directed by unit procedures or downtime processes. Follows all departmental downtime procedures. Able to perform all regular duties when the hospital computer system is not available.


Prepares and maintains patient's charts. Reviews charts at the end of every shift to ensure adequate supply of forms are filed in the chart and that the chart is maintained in proper order. Ensures that after discharge the patient's chart is completely assembled for Medical Records. Prepares teh chart in proper order for patients transferred to another unit.

Accurately and legibly maintains staffing, record of admissions and discharges, sick calls, floating, and docking per established unit process.

Notifies Case Management (when on unit), when orders are written for discharge or discharge planning (i.e. placement, durable medical equipment, home health, acute transfers).

Obtains Medical Records for physicians from other facilities.

Arranges for a follow-up appointment with Primary Care Physicians (PCP). Refers to Case management if they do not have a PCP.

Files all hard copy medical records appropriately in the patient chart, i.e. lab results, progress notes, operating room records, EKG strips, downtime order forms.

Copies chart and all materials as directed and distributes to appropriate people.

Is an active participant in achieving quality patient care.

Refers complaints and problems to the Nurse Manager, Assistant Nurse Manager or Team Leader.  Holds self and other team members accountable to maintain standards.

Maintains confidentiality with all patient care issues.

Answers patient or family questions or refers them to the appropriate personnel.

Answers patient call light and directs requests to the appropriate person.  Ensures the patients request is received by a person who can assist them timely. Answers phone within four rings.

Keeps work area and nurses’ station clean and well organized.

Orients students, new unit staff, and student volunteers to the unit's clerical function. Mentors new hires on computer usage appropriate to their position/unit.

Assists nursing staff in achieving quality patient care by transporting specimens and retrieving items needed for patient care. Is certified to pick up blood from the Lab.

Performs data collection for audits and other performance improvement initiatives as delegated by the unit leader or charge nurse. Utilizes the electronic health record reports and dashboard (i.e. Clinical Leader Organizer) to monitor and collect information as assigned.

Reads and complies with current hospital/patient care procedures and policies as applicable.

Unethical and illegal professional activities shall be reported to the appropriate authorities.

Pay Range

$20.38 -$30.57
