POSITION SUMMARY:
The Quality Management Specialist plays a key role in assuring successful implementation of RWPA and RWPC programs by monitoring and assessing progress of key clinical quality management (CQM) indicators along the HIV continuum of care.
The Quality Management Specialist will work closely with the HIV Care and Prevention Clinical Quality Management team to plan, track, and assess clinical metrics of
success for PLWH. Working closely with the Senior Director of Programs, the Quality Management Specialist will support the delivery of high-quality comprehensive care for all PLWH. Additional tasks include identfying patient’s progress, gaps in services, and any needs for technical assistance.
Moreover, the QMS will lead and complete annual, quarterly, and monthly reports for HRSA and other agencies , these include but are not limited to: HIV Qual requirements, RSR, CAREWare updates, and organizational treatment cascades.
RESPONSIBILITIES:
· Monitor the implementation of RWPA and RWPC funded services by tracking and reviewing data submitted by RW staff.
· Assure timely completion and submission of RSR, Organizational Cascade, progress updates and required reporting to HRSA, and NYC DOHMH.
· Monitor, track, and report progress and challenges of HIV Care programs.
· Maintain patient lists ( such viral load, retention, lost to care) and other data collection and tracking tools monitoring metrics along the HIV continuum of Care.
· Identify gaps in services and support the delivery of technical assistance for RW programs.
· Track patient’s progress to ensure patient-centered quality of care.
· Monitor and track key metrics of PrEP education, counseling, and access.
· Complete annual report for IPRO
· Attend scheduled trainings to stay current on changes in CAREWare, eCW, and Clinical
Quality Management.
· Assure completion of data entry in CAREWare and eCW for RW.
· Complete annual report to satisfy Ryan White Grant program requirements
· Assist Senior Director of Programs on special projects
· Perform other duties as assigned
Qualifications:
· Knowledge and familiarity with Q/I tools (PDSA cycles, flowcharts, Force-Field Analysis (SWOT), etc.)
· Knowledge of computers (Word, Excel, Microsoft Outlook) and office equipment.
·Knowledge and experience with HRSA clinical quality management measures and reporting requirements (perferred)
· Must demonstrate broad understanding of HIV epidemic (history, priority populations, transmission, prevention, treatment, sociopolitical issues, and quality improvement).
