Key Role Responsibilities:

  • Lead discharge planning and coordinate post-hospital care for patients within 30 days after discharge, collaborating with a multidisciplinary team to ensure the best outcomes.

  • Assess patient needs, determine appropriate levels of care, and facilitate smooth transitions to the next phase of recovery.

  • Work with insurance providers to secure coverage and support patient education and preventive care programs.

  • Play a key role in reducing readmissions and improving patient satisfaction through effective communication and planning.

Qualifications:

  • Requires an Associates of Nursing (ASN).
  • Bachelors of Nursing (BSN) preferred.
  • Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
  • Requires that the RN has graduated from a nationally accredited nursing program.
  • Requires 3-5 years of relevant experience.
  • Requires Basic Life Support certification through the AHA.
  • Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
  • Requires proficiency in Microsoft Office and applications.
  • Requires understanding of medical record requirements, regulations and policies.
