As a Physical Therapist, you will provide in-home evaluations, treatments, and education to help patients improve strength, mobility, safety, and independence. You will work closely with nurses, occupational therapists, speech therapists, and other members of the interdisciplinary team to ensure comprehensive, patient-centered care. Full-Time and Part-Time Positions Available Benefits of Joining Iowa Home Care Flexible scheduling to support work-life balance  Health,  Dental, Vision and Life Insurance Short-term  and Long-Term Disability plans Flex-Spending Paid Mileage or Company Sponsored Vehicle Employee Assistance Program Excellent 401K with Employer Match PTO and Paid Holidays Wellness Program  Continuing Education opportunities Supportive clinical team environment with strong leadership and dedicated case management  Autonomy, professional respect, and opportunities for growth within a trusted, locally owned agency The opportunity to make a real difference by helping Iowans remain safely in the place they love — their home Duties to include:  • Conduct comprehensive in-home physical therapy evaluations • Develop and implement individualized treatment plans focused on mobility, strength, balance, and safety • Provide therapeutic interventions including exercise programs, gait training, transfer training, and pain management • Educate patients and caregivers on home exercise programs, safety strategies, and equipment use • Collaborate with nurses, occupational therapists, speech therapists, social workers, and case managers to coordinate care • Document all patient visits, progress notes, and care plans in compliance with agency and regulatory standards • Recommend adaptive equipment and home modifications when appropriate • Monitor patient progress and adjust treatment plans as needed • Communicate changes in condition or concerns to the interdisciplinary care team • Promote independence and ongoing improvement through compassionate, patient-centered care   Our Values at Iowa Home Care Professional – We set high standards for quality and professionalism in everything we do. Minimum of two years of experience in physical therapy; community/home health experience preferred. Strong communication, organizational, and problem-solving skills. Valid driver’s license with reliable transportation.
