Home Health Physical Therapist (PT)


Job Summary:

Provide skilled physical therapy services to patients in their homes, promoting mobility, independence, and overall quality of life through personalized treatment plans and education.

Key Responsibilities:

  • Conduct comprehensive physical therapy evaluations in patients’ homes.
  • Develop individualized treatment plans based on physician orders, assessment findings, and patient goals.
  • Deliver evidence-based therapeutic interventions to improve strength, balance, mobility, and functional independence.
  • Educate patients, caregivers, and families on home exercise programs, safety techniques, and assistive device use.
  • Monitor and document patient progress, response to treatment, and any changes in condition.
  • Communicate regularly with physicians, nurses, and other interdisciplinary team members to coordinate care.
  • Ensure compliance with agency policies, state practice acts, and federal regulations.
  • Maintain accurate and timely clinical documentation according to agency and payer requirements.
  • Provide input for discharge planning and recommend continuation of therapy or community resources as appropriate.
  • Participate in case conferences, quality improvement activities, and ongoing professional development.

Qualifications:

  • Licensed Physical Therapist (PT) in Michigan.
  • CPR certification
  • Minimum 1 years of clinical experience, preferably within home health in the last 5 years.
  • Valid driver’s license, reliable transportation, and proof of insurance.
  • Strong clinical reasoning, communication, and time-management skills.
  • Compassionate, patient-centered approach with the ability to work independently.
