SUMMARY: The LPN uses experience, knowledge, judgment and skills to provide nursing care to patients and families in all settings. Maintains patient confidentiality and promotes patient rights as supported by agency, state and federal directives. Identifies cultural and lifestyle choices and non-judgmentally accommodates for such differences. The LPN is committed to the philosophy, goals and objectives of the hospice concept. The LPN works under the direct supervision of the RN while consistently demonstrating the Hospice Savannah values and compliance with organization’s service excellence standards.
QualificationsQUALIFICATIONS: Graduate of an accredited school of Nursing, with a current GA LPN License. Minimum of two years of experience in hospice, oncology, med-surg or home health providing direct patient care required. Current CPR certification, valid driver’s license, automobile insurance and reliable transportation required. Excellent communication skills (written and verbal) and the ability to successfully interact with a diverse patient population, families, physicians, Hospice Savannah staff, and the community at-large along with excellent customer service and documentation skills required. Computer proficiency in Microsoft Office products including Outlook, and database knowledge required.
PHYSICAL DEMANDS: Position requires moderate visual concentration for patient charting/documentation and computer work. Position carries a risk of exposure to infectious diseases and requires skill in standard safety precautions. The LPN requires the mobility to ambulate or lift a patient with or without the assistance of devices in excess of 75 lbs. utilizing proper body mechanics.
WORK ENVIRONMENT: This part-time position works a 20-24 hour workweek, rotating days, evenings, or weekends, with occasional requests to work additional days/hours/holidays as needed. At times, due to patient census and acuity, this position may be required to remain on site at Hospice House during meal breaks. The Admissions LPN position may be required to provide care in what has been determined by local authorities as a high-risk crime area and unsafe situations (potential or actual). The LPN is responsible for providing care under the direction of the RN and must be able to accurately, timely and concisely document the patient/family encounter using a computer. This position encounters stressful situation with patients and their families regarding end of life care. The Admissions LPN will be required to perform patient visits utilizing their personal vehicle, and may require extensive driving.

Hospice Savannah, Inc. is a not for profit hospice founded by the community in 1979, offering a continuum of care from palliative support and caregiver support following the initial diagnosis of a serious progressive illness; hospice care when there is a prognosis of six months or less; and bereavement support for patients’ loved ones and for anyone in the community.
The only hospice in the Coastal Empire to be accredited by the Joint Commission we serve patients throughout Bryan, Chatham, Effingham, Long and Liberty counties. It is a member agency of the United Way of the Coastal Empire, the National Partnership for Hospice Innovation and the Georgia Hospice and Palliative Care Organization. It has affiliations with many academic programs in the Savannah area and offers education and residency opportunities for nurses, medical residents, social workers and more.
The homelike inpatient facility located at 1352 Eisenhower Drive is available to hospice patients for medical issues that cannot be resolved at home and provides respite for weary caregivers. Under the guidance of full-time medical directors, board-certified in hospice and palliative care, more than 125 volunteers and 125 full and part-time staff are committed to providing extraordinary care.
The Demere Center for Living at 6000 Business Center Drive is home to the Hospice Savannah Foundation and community-supported programming: Full Circle Grief and Loss; the Steward Center for Palliative Care and the Edel Caregiver Institute. The Foundation funds a full-time music therapy program and full-time massage therapy program and the hospice offers robust “We Honor Veterans” and “Pet Peace of Mind” programs.
