This position covers Lancaster, York and surrounding counties.&nbsp;</span></p> <p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Orientation: 5 weeks full time, 8am - 5pm&nbsp;</span></p> <p>&nbsp;</p> <p><span style="text-decoration: underline; font-family: arial, helvetica, sans-serif; font-size: 12pt;"><strong>PRN Requirements</strong></span></p> <p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">2 shifts/month.</span></p> <p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">1 weekend per quarter</span></p> <p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">1 holiday every 2 years&nbsp;</span></p> <p>&nbsp;</p> <p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;"><strong>Essential Functions</strong></span></p> <p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Provides accurate and timely information regarding hospice services to anyone inquiring about Hospice care.</span></p> <p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Completes admission visits for routine, general inpatient (GIP) and Hospice Response Team (HRT) patients.</span></p> <p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Performs RN comprehensive assessments and focused skilled nursing visits on GIP/HRT per discipline frequencies.</span></p> <p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Follows up on hospice waiting patients&nbsp; to assure a smooth hospital discharge.</span></p> <p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Documents in both the hospital EHR and hospice EMR in an accurate and concise manner at the time the service is provided.
