Duties:
- Provide direct clinical care to a diverse patient panel in an outpatient clinic setting, ensuring comprehensive care across a range of health conditions.
- Perform health assessments, including physical exams, diagnostic tests, and treatment plans, while managing acute and chronic health conditions.
- Administer immunizations, prescribe medications, and provide ongoing health education to patients.
- Conduct preventative care services, including health screenings, wellness checks, and chronic disease management.
- Collaborate with physicians, specialists, and other healthcare providers in managing patient care, referring patients as necessary for specialized services.
- Participate in patient care management for individuals of all ages, including pediatric, adult, and geriatric populations.
- Provide culturally sensitive care while addressing social determinants of health and working with patients from underserved communities.
- Work collaboratively with multidisciplinary teams, including MAs, nurses, case managers, and behavioral health specialists, to provide holistic patient care.
- Participate in community outreach programs to promote health education, disease prevention, and health awareness.
- Attend all relevant meetings as necessary
- Document all patient encounters in the Electronic Health Record (EHR) accurately and in a timely manner.
- Ensure compliance with healthcare policies, procedures, and regulations
- Maintain continuing education and professional development to stay current with evidence-based practices and clinical guidelines.
- Act as a role model for the healthcare team in terms of professionalism, patient care, and communication.
- Adhere to the practice of confidentiality regarding patients, families, staff and the organization.
- MD/DO from an accredited educational institution
- Unrestricted license to practice medicine in the State of IL
- BC/BE Family Medicine or Internal Medicine Physician
- We will consider recent grads
- Full time (M-F 8-5)
- $245,000 - $290,000 (based on experience) plus potential bonus compensation
- PTO/Vacation 3 weeks to start
