Reports to: Program Director

Status: Non-exempt

Grade: A04

Salary: Dependent on Qualifications

Position Summary:

The Respite Liaison provides administrative and client-focused support for the respite care program. This role is responsible for tracking medical appointments, coordinating transportation for respite clients, and serving as the central point of contact between partnering hospitals, clients, and assigned case managers. The Liaison ensures client satisfaction through consistent follow-up and communication.


Essential Duties & Responsibilities:

  • Coordinate with hospitals, doctors’ offices, and community providers to secure resident appointments.

  • Maintain a daily, weekly, and monthly calendar of all resident appointments.

  • Manage and provide transportation when needed for residents to medical and service-related appointments.

  • Maintain accurate resident counts and documentation

  • Conduct daily check-ins with residents

  • Advocate for resident needs and address concerns promptly

  • Maintain regular communication with AOC case managers

  • Enforce resident policies and procedures

  • Perform other duties as assigned

Qualifications:

To perform this job successfully, an individual must have excellent time management skills, attention to detail, and excellent communication and interpersonal skills. Additionally, an individual must be able to think strategically and act quickly. Must possess the ability to work well with others across a broad spectrum of situations. Prior experience in working with individuals experiencing homelessness and mental illness is required.

  • High School Diploma or GED

  • A Minimum of four (4) years of experience working with the unhoused population

  • Practical experience in customer service or a related field

  • Prior experience working with individuals experiencing homelessness and mental illness

  • Training in de-escalation and trauma-informed care

  • Excellent verbal and written communication skills

  • Strong interpersonal skills

  • Excellent driving record

  • Able to efficiently navigate driving in the Albuquerque area.

  • Ability to build rapport with diverse populations

  • Ability to work collaboratively with residents, staff, and service providers

  • Strong organizational and time-management skills

  • Ability to manage multiple tasks and meet deadlines

  • Attention to detail and ability to maintain accurate records

  • Ability to remain composed in challenging situations

  • Proficiency in Windows, Word, and Outlook

  • Experience with data entry and file management

  • Reliable and dependable

  • Maintain a clean, organized workspace

Other Requirements:

  • Valid Driver’s License and reliable transportation.

  • Ability to navigate stairs, ladders, ramps, and uneven terrain.

  • Must become CPR/First Aid trained within 90 days of hire.

Heading Home is a New Mexico non-profit that provides shelter and housing services to people experiencing homelessness. We work to make experiences of homelessness as rare, short-lived and nonrecurring in our community. We fulfill this vision through serving as a unified voice of advocacy, prevention and service; we are an innovative convener, collaborator and educator about homelessness in the community. We are dedicated to serving those most in need in New Mexico by providing emergency and permanent supportive housing with access to wrap around services. Our programs serve families, women, men & veterans. Heading Home operates two shelters – the Albuquerque Opportunity Center and the Westside Emergency Housing Center. We run a medical respite program which provides 24-hour care to men recently released from the hospital who do not have a home. Our signature Housing First program, Albuquerque Heading Home, has housed hundreds of medically vulnerable, chronically homeless individuals since 2011. Our programs for families provide rental assistance and permanent supportive housing and case management. All of our programs assist veterans in exiting homelessness into permanent supportive housing. We utilize the Housing First, Collective Impact and Harm Reduction models to provide life-saving services to some of the most vulnerable members of our community.
