The Case Manager is responsible for providing a range of services for clients with either Psychiatric and/or Substance Abuse issues. Also provides advocacy, outreach, and service linkage for clients and initiates access to entitlement services.
• Bachelor's degree in Human Services or related field, Social Work preferred
• Valid driver’s license may be required based on program
• Knowledge of community resources preferred.
• Computer proficiency and keyboarding skills required
