PURPOSE:

Provides leadership and organization of Personal Care Attendants to ensure the holistic care and support of Weston residents. Supports the Director of Weston Residential Terrace in the scheduling of services and staff. The Resident Care Coordinator is also responsible for coordinating and supporting home services program.

ESSENTIAL JOB DUTIES:

1. Assists with the orientation of new PCA staff and monitors PCA performance on an on-going basis to ensure compliance with individual resident service plans and Department policies and procedures.

2. Provides overall supervision of resident living units and common areas to ensure tidiness and absence of safety hazards.

3. Ensures all activities on individual resident service plans are performed as scheduled in the proper manner with additions and comments noted when changing circumstances are identified.

4. Encourages a positive relationship between residents, family, and staff by responding to needs in a timely manner and providing information/education as indicated.

6. Actively participate in appropriate problem-solving activities keeping the Director informed of any concern that may impact the well-being of a resident, family understanding, Department policy, and regulatory compliance.

7. Provide direct care services to residents within the scope of the Personal Care Attendant job description.

8. Participate in quality review activities, summarize data, and suggest recommendations for improvement to the Director.

9. Assists with the preparation and management of PCA staff schedules to be approved by the Director to assure consistent and sufficient staffing to meet Residents needs.

10. In coordination with Health and Wellness Navigator, identify and develop programs to promote physical well-being and independence through home services program;

11. Development of Resident/client’s home service plan;

12. Provision of care and services according to Resident/client’s home service plan;

13. Delegation of home services interventions to appropriate individuals as needed;

14. Observation of Resident/client functioning and addressing changes through coordination with Resident, families, physicians, physical therapy or other providers;

15. Completion of appropriate records documenting service provision;

16. Promotion of the Home Services Program through educational offerings, marketing/sales visits, other activities;

17. Assumes on call duties as directed by the Director

18. Keep the Director informed of all significant matters at the time of occurrence.

19. Assist with presenting staff in-service programs.

20. Demonstrate initiative in any matter that will promote the improved quality of services to our customers.

21. All other duties as assigned.

QUALIFICATIONS

  • Illinois certification as a Nursing Assistant, LPN, or Medical Assistant
  • Current CPR certification
  • A minimum of five (5) years experience working with the elderly in a caregiving environment preferred.
  • Previous supervisory and scheduling experience desired. Supervisory skills involving good communication
  • Scheduling experience a plus. Good organizational abilities.
  • Experience leading co-workers, good problem-solving and CQI abilities
  • Familiar with established protocols and regulations in order to assist management operations ensuring accuracy and delivery of services.
  • Job responsibilities typically require considerable judgment and problem-solving in the interpretation and analysis of many diverse facts and issues and the application of appropriate policies, guidelines, clinical protocols, and/or precedents. Ingenuity and foresight are often required in the absence of established guidelines or procedures.
  • Valid driver’s license required.

PHYSCIAL REQUIREMENTS/WORKING CONDITIONS:

Have the ability to move throughout the facility promptly, especially in emergency situations. Must be able to lift up to 50 lbs. unassisted, and up to 150 lbs. with the assistance of one. Must

be able to stoop, bend, and reach; must be able to push and pull up to 150 lbs. in a wheelchair; good visual and hearing acuity required; Good verbal communication skills required. Must be able to walk or stand for up to 80% of the workday. Must be free of communicable disease; may be exposed to communicable diseases, and blood and body fluids that may contain HIV and/or HBV. May be exposed to cleaning chemicals and offensive odors.

Working the majority of time in well lighted, comfortable and ventilated rooms. Frequent stooping, reaching, standing, walking, pushing and pulling. Work is of medium demand and requires consistent lifting up to 10 lbs., frequent lifting up to 20 lbs. and occasional lifting over 75 lbs. Near visual acuity to read documents, measure devices/gauges, and operate other machines. Performs tasks requiring independent knowledge in addition to procedures or instructions provided. Worker subject to both inside and outside environmental conditions. Potential for exposure to blood borne path
