About this role

Job Summary:

Provides individualized assistance to oncology patients, families, and caregivers to identify and minimize barriers to care. Promotes achievement of organizational and departmental objectives and strategic plan.

Definition of Shared Governance: Shared Governance is a professional structural model, founded on the cornerstone principles of partnership, equity, accountability, and ownership that form a culturally sensitive and empowering framework, enabling sustainable and accountability-based decisions to support an interdisciplinary design for excellent patient care.

Essential Job Duties:

  1. Facilitates the patient's timely access to medical and psychosocial care across the continuum from screening, diagnosis, treatment, rehabilitation, survivorship and/or end of life.
  2. Coordinates a seamless model of access and care benefiting patients, family members, and participating physicians throughout the GSH system including both primary and secondary service areas.
  3. Utilizes a collaborative, interdisciplinary approach to maximize utilization of resources and minimize duplication of services.
  4. Maintains effective and timely communication with all members of the patient's healthcare team.
  5. Actively seeks and participates in educational opportunities, both formal and informal, to achieve and maintain knowledge and skill related to job responsibilities.
  6. Attends patient care conferences, and other cancer program meetings as appropriate.
  7. Facilitates interdisciplinary care planning meetings as appropriate.
  8. Participates in planning, implementation, and evaluation of community needs assessment at least once every three years.
  9. Collects data, documents, and provides report to Cancer Committee on an annual basis to include: identified health disparities, description of navigation process, populations served and barriers identified, documentation of activities and metrics (outcomes), and areas for PI/QI enhancement and future directions.
  10. Maintains documentation related to navigation in appropriate eMR (Magview, EPIC, etc.)
  11. Apply for nursing council or committee and be vested on one council at least every 5 years.  80% attendance in person or by phone is required.  Advancements in Clinical Ladder will be reflected in yearly application process and is expected for job performance standards.

Secondary Job Duties That May be Reassigned:

  1. Represent Oncology Program in community activities
  2. Assists with marketing strategies to help promote program
  3. Breast Navigator will also be responsible for assisting physician in Breast Center as designated by GSPN Operations Manager.
  4. Breast Navigator will cross train to provide nursing coverage to the Breast Care Center.

 

Qualifications

Job Specifications:

  • Experience: Practical work experience in oncology with preferred experience in outpatient oncology and working with financial aspects of cancer care (CMS, third party payors, etc.).
  • Obtain and maintain applicable certification within one year of becoming eligible.
  • Licensure - Registered Nurse or Registered Radiologic Technologist
  • Certification - Breast Health Clinical Navigator Certification required within 1 year of employment
