About this role

What you will do

  • Provide on-call psychosocial support to hospice patients, caregivers, and families by phone and in person.
  • Respond promptly during assigned on-call shifts and review reports, emails, and messages at shift start.
  • Provide crisis intervention, grief support, education, and counseling per the hospice plan of care.
  • Support admissions, changes in level of care, and end-of-life needs in collaboration with the after-hours care team.
  • Complete accurate, timely documentation and shift reports per agency standards.

What we are looking for:

  • Master’s degree in social work (from a school of social work accredited by the Council of Social Work Education); LCSW or licensure eligible preferred;
  • At least three years of social work or counseling experience; hospice or end-of-life care preferred.
  • Strong skills in crisis intervention, grief support, case management, and critical thinking.
  • Excellent communication, collaboration, and ability to work independently.
  • Compassionate, emotionally mature, and mission-driven with a heart for end-of-life care.

Schedule & Compensation

Schedule: PRN (as-needed) position. Team members are required to pick up a minimum of 15-20 shifts per month. Available shift options include:

Weekdays: 5:00 PM – 8:30 AM

Weekends: 8:30 AM – 5:00 PM (day) 5:00 PM – 8:30 AM (overnight)

Flexibility to work a mix of weekday and weekend shifts is expected.

Benefits: 401(k) with company match (100% match on the first 3%, 50% match on the next 1%)
