Summary:

As a part of the Goldbelt Glacier Team, you will be supporting medical services for active-duty military personnel, their dependents, eligible DoD civilian employees, and other eligible beneficiaries designated by the Government. As a HealthCare Worker (HCW). 


Responsibilities

Essential Job Functions:

  • Practice as an interdependent member of the health team and provide important components of primary health care through direct social work services, consultation, collaboration, referral, teaching, and advocacy. 
  • Provide direct and indirect services to both inpatients and outpatients in accordance with social work standards of practice. 
  • Assess and treat outpatients in individual, group, couples, and family modalities exercising professional judgment and using a wide range of social work skills to include individual and family counseling to assist patients and their families in dealing with chronic and acute diseases/injuries. 
  • Conduct psychosocial assessments to determine patient needs and resources (both family and community support).  
  • Provide counseling to patient and family in matters directly related to patient limitations, adjustment to medical condition, and ongoing treatment.  
  • Develop and implement discharge plans, follow-up care, and transfers to other health care facilities (e.g., nursing homes, rehabilitation hospitals, and VA facilities). 
  • Plan and maintain referral and coordination services with civilian health and social service agencies to provide optimal patient care. 
  • Provide consultation services to medical, nursing, and ancillary hospital staff regarding psychosocial issues, discharge plans, and follow-up care for patients and families. 
  • Provide crisis intervention services. 
  • Respond independently, and with various media, to appropriate military and community requests for lectures and presentations.  
  • Take the initiative in seeking out opportunities to present programs to meet the needs of service members and their families. 
  • Develop and maintain working relationships with community resources.  
  • Coordinate with teachers, lawyers, physicians, and representatives of their service disciplines for the benefit of the service member and families.  
  • Take initiative in identifying and assessing the needs of the military community, and organize responses to address those needs. 
  • The HCW is responsible for a full range of diagnostic examinations, the development of comprehensive treatment plans when indicated, delivery of treatment within the personnel and equipment capabilities of the treatment facility, provision of mandated medical surveillance and preventive services, and the quality and timeliness of treatment records and reports required to document procedures performed and care provided.  
  • The HCW shall refer patients to staff specialists for consultation opinions and continuation of care and shall see the patients of other government staff health care providers who have been referred for consultation and treatment.  
  • The HCW’s productivity is expected to be comparable to that of other Licensed Clinical Social Worker assigned to the same facility and authorized the same scope of practice. Services provided under this contract comply with the National Association for Social Workers (NASWs’) Standards for Clinical Social Work; Standards of the Joint Commission on Accreditation of Healthcare Organizations; and other applicable credentialing and accreditation agencies and applicable provisions of law and the rules and regulations of any and all governmental authorities pertaining to: (a) Licensure and/or regulation of healthcare personnel in treatment facilities, and (b) the regulations and standards of professional practice of the treatment facility, and (c) the bylaws of the treatment facility’s professional staff. 
  • Perform a psychosocial assessment of the family situation, which may include, but is not limited to administration, scoring and interpreting family evaluation and adaptive behavior scales to family members; review of medical, mental health, educational, and other records as are available; Assessment of patients and families who have social, psychological and/or environmental needs related to weight management or associated co-morbidities.  
  • Essential duties include, but are not limited to, interviewing patients and families, developing plans for intervention addressing agreed upon priorities, Conducts intervention programs, as needed, to include biofeedback therapy and other modalities to support Life Management Counseling (LMC) programs. Performs medical intake (surveys, history, intake eval, health promotion, tobacco cessation) and assignment of beneficiaries LMC programs, assisting patients/families in improving or restoring their capacity for a healthy and balanced lifestyle. 
  • Services shall be provided for interviewing, assessing and treating behavioral health difficulties experienced by active duty military personnel, their dependents, eligible DoD civilian employees, and other eligible beneficiaries, designated by the Government. 
  • Provide individual, group and family therapy services to all beneficiaries. 
  • Provide mental health services, as a direct service provider or as a consultant. Contact Unit Medical officers or Line officers with recommendations for dispositional determinations on patient care when appropriate. 
  • Prepare written reports for medical records of the assessment, diagnosis and (psychosocial) treatment plan.  
  • Systematically documents patient contacts and progress in accordance with Directorate and USNH command policy.  
  • Make appropriate referrals to other medical and mental health professionals as indicated. 
  • Become familiar with and demonstrate awareness of the Bylaws of the Medical Staff and the organizational and operational policies of the MTF and comply therewith. 
  • Become familiar with the Department of Defense TRICARE Program and the methodology to function therein. 
  • Joint Commission requirements - Comply with the standards of the Joint Commission, applicable provisions of law and the rules and regulations of all governmental authorities pertaining to licensure and/or regulation of healthcare personnel in treatment facilities. 
  • Consult with Naval Hospital Command and administration, supplying information and feedback regarding procedures and services provided by the Psychological Divisions. 
  • Act as a human services broker, using clinical judgment and knowledge of area resources to provide information and referrals to patients and other care providers. 
  • The HCW is required to maintain Basic Life Support Level C re-certification during the term of the contract. This re-certification will be provided by the Navy. 

Qualifications

Necessary Skills and Knowledge:

  • Maintain a level of productivity comparable with that of other individuals performing similar services.  
  • Adhere to and comply with the Bureau of Medicine and Surgery, and local instructions and notices in effect during the term of the contract.  
  • Practice aseptic techniques as necessary; comply with infection control guidelines to include the proper handling, storage, and disposal of infectious wastes, and the use of universal precautions to prevent the spread of infection. Function with an awareness and application of safety procedures.  
  • Apply an awareness of legal issues in all aspects of patient care and strive to manage situations in a reduced risk manner.  
  • Exercise awareness and sensitivity to patient/significant others' rights, as identified within the MTF.  
  • Maintain an awareness of responsibility and accountability for own professional practice.  
  • Maintain DoD email account as directed by government supervisor.  
  • Exercise appropriate delegation of tasks and duties in the direction and coordination of health care team members, patient care, and clinic activities.  
  • Be able to comply with all applicable Federal, State, and local laws, Department of Defense, Bureau of Medicine and Surgery, and MTF instructions and policies. Proficiency in Microsoft Office software applications including Word, Excel, Access, PowerPoint, and Outlook. 

Minimum Qualifications:

  • Possess a master’s degree in social work (MSW) from a program accredited by the Council on Social Work Education (CSWE). 
  • Possess a current, unrestricted license to practice independently as a Clinical Social Worker in any one of the fifty States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands. 
  • Have a minimum of two years Post Masters experience in behavioral healthcare setting as a full- time Licensed Clinical Social Worker within the last 3year(s). 

Preferred Qualifications:

  • A thorough understanding of the military lifestyle and experience working with military family members. 
  • Working knowledge of relevant military, state, federal, and local laws and resources. 
  • Highly developed written, oral, and presentation communication skills.
