Job Summary:

The Addiction Medicine Advanced Practitioner Training Program will provide integrated mental health and SUD/OUD services, including Medications for Opioid Use Disorder (MOUD) for PAs and advanced practice nurses (APNs) , in a primary care underserved community-based setting. The overall goal of the APP Trainee Program is to foster robust clinical training and augment expertise among clinicians who will see patients at access points of care and provide mental health and addiction prevention, treatment, and recovery.

Job Duties:

Trainee will participate in MOUD in community-based settings along with additional clinical training beyond the didactic component, including professional peer-to-peer support to assist trained APNs and PAs in initiating their provision of MOUD.

Trainee will connect and manage patients’ receiving MOUD in receiving behavioral health counseling and in connecting to social supports to retain patients in care.

Trainee will collaborate with care givers inside and outside of the immediate care team.

Trainee will be able to demonstrate clearly and effective communication with patients and immediate care team.

  • Initial and ongoing assessment of patient's medical, physical, and psychosocial status.
  • Perform history and physical.
  • Develop assessment and treatment plan.
  • Participate in clinical patient care.
  • Record progress notes.
  • Order tests, examinations, medications, and therapies.
  • Arrange for discharge and after care.
  • Write and dictate admission notes, progress notes, procedure notes, and discharge summaries.
  • Provide patient education and counseling covering health status, test results, disease processes, and discharge planning.
  • Perform procedures.
