Works with, and assists, other CRNP in the performance of Face to Face Encounters and in the evaluation of the Expanded Care Program requests. In coordination with the Intake/Referral Center, conducts the Medical Records Abstraction process. Serves as primary care provider for selected patients referred by GHC and Gilchrist Greater Living who are in need of home-based medical care.Education: Successful completion of certified nurse practitioner program. Licensures/Certifications: Current licensure as an adult nurse practitioner by the Board of Medical Examiners of Maryland/Maryland State Board of Examiners of Nurses. Completion of health care CPR course annually. Experience: 5 years of experience functioning as a nurse practitioner in a clinical setting. Prior experience in a clinical hospice role is desired. Prior experience in a quality assurance or utilization review role is desirable. Skills: • Broad knowledge of medical treatment practices, especially internal medicine, geriatrics, and oncology, • Knowledge of healthcare regulatory agency standards for hospice/ health care organizations. • Knowledge of The Joint Commission (TJC) and CMS principles and standards. • Familiarity with Part A Hospice Medicare and insurance coverage processes preferred. • Ability to operate independently and to balance multiple priorities. • Strong leadership, interpersonal, and influencing skills necessary to interact effectively with physicians, payers, regulatory agencies, staff, and other health professionals. • Skill in using computers and personal productivity applications. • Skill in written and oral communication. • Ability to interact effectively with patients and families. • Skill in reviewing, analyzing, and developing clinical summaries of patient electronic medical records.

Pay Range

$82,774.65 - $137,823.94

Final salary offer will be based on the candidate's qualifications, education, experience and alignment with our organizational needs.
