The Care Navigator will support and promote FBH’s vision and mission to enhance and change lives. The Care Navigator, through their interactions with the clients we serve and in collaborating with community partner agencies, will improve the quality of care for individuals by providing referrals and linkages to resources which will enhance the services they are receiving at FBH and will support their overall recovery and wellness.

ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDED BUT ARE NOT LIMITED TO THE FOLLOWING:

  • Engage individuals enrolled in services at FBH and provide referrals to primary health care, dental care, or other services offered through FBH partnerships and contracts.

  • Access and utilize, per partnership various EMRs, scheduling platforms, and systems in coordinating and scheduling appointments for individuals.

  • Demonstrate a positive attitude and efforts toward meeting the needs of internal and external customers.

  • Must be able to apply active listening techniques conveying respect, professionalism, engagement skills, positive attitude and empathy.

  • Effectively utilize customer service and motivational interviewing skills to promote appropriate client-centered care.

  • Demonstrate flexibility in responding to change and in working with diverse groups and individuals.

  • Promote reducing barriers to services including coordination to arrange and schedule appointments. May need to meet individuals at appointments as necessary to engage in the program.

  • Must possess excellent communication skills including the ability to send and receive oral and written messages, in a clear, positive, prompt and appropriate manner.

  • Have flexibility in learning multiple operating systems and processes by various organizations and demonstrate the ability to work in an EMR, including the dependence of, translation of, and utilization of electronic reports.

  • Demonstrate promptness and reliability in attendance, work deadlines and in performing tasks. Complete all assignments without prompting.

  • Support and encourage a harmonious working relationship within and between departments.

  • Requires travel to see clients in the community and travel between work sites.

  • Other duties as assigned.

QUALIFIED CANDIDATES WILL HAVE:

REQUIRED:

  • High school graduate or equivalent.

  • Customer Service experience.

  • Must have an unrestricted and valid driver’s license, active car insurance and must be willing to use their personal vehicle in the course of work. 

PREFERRED:

  • 2 years general office experience.

KNOWLEDGE, SKILLS & ABILITIES:

  • Must possess excellent communication skills including the ability to send and receive oral and written messages, in a clear, positive, prompt and appropriate manner, while maintaining customer confidentiality.

  • Must be able to apply active listening techniques conveying respect, professionalism, engagement skills, positive attitude and empathy.

PHYSICAL, SENSORY & ENVIRONMENTAL:

The demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.  Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions.

  • Work is generally performed in an office setting which may involve sitting and use of computers for four to eight hours per day. 

  • May be required to lift 10 - 20 pounds.

  • Work may be performed out of office and travel may be needed to outside areas.

  • Blood-borne Pathogen Category III - No predictable exposure to blood-borne pathogens.

PAY RANGE (Hourly/ Non-Exempt): $21.89- 26.65

All pay is based on longevity at Frontier Behavioral Health; thus, the starting pay for this position is $21.89 per hour.

Benefits described above are based on employees who hold .5 FTE positions or greater and/or some benefits vary based on FTE. PRN positions are not benefit eligible or eligible for a sign on bonus.

About Frontier Behavioral Health

Frontier Behavioral Health (FBH) was formed in 2011 with the merger of two long-standing organizations: Spokane Mental Health (since 1970) and Family Service Spokane (since 1905). Combined, FBH has more than 100 years of experience providing behavioral healthcare services in Spokane County. FBH is the lead service provider of Crisis Response Services in the region and provides 24-hour crisis intervention services and Involuntary Treatment Act evaluations. FBH serves more than 16,000 clients a year with a staff of more than 700 and provides a full range of services to people of all ages including Children and Youth, Adults, and Elders. While the majority of services are provided in an outpatient setting, FBH also operates 48 inpatient beds where Evaluation & Treatment, and Stabilization services are provided. Other services provided by FBH include Peer Support, Crisis Services, services in Nursing Homes, Schools, and for the Homeless. FBH also operates a Clubhouse and Vocational Rehab/Employment services.
