<br><strong><u>Summary</u></strong>
</div><div>We are seeking a Licensed Clinical Social Worker (LCSW) to join our interdisciplinary pain management team. This role is vital in supporting patients’ emotional, psychological, and social well-being as part of a structured chronic pain treatment program.<br> <br><strong><em>Candidates with experience in pain management, hospice, or palliative care are strongly encouraged to apply.</em></strong> This is an outpatient role with predictable hours, no weekends, and no on-call responsibilities.<br><br>

<br><strong><u>Responsibilities</u></strong>
</div><ul>
<li>Conduct individual clinical sessions addressing chronic pain and mental health needs.</li>
<li>Provide evidence-based therapy including CBT, ACT, and mindfulness-based approaches.</li>
<li>Monitor patients for changes in mood, cognition, or behavior related to medication therapy.</li>
<li>Conduct psychosocial assessments including trauma history and social determinants of health.</li>
<li>Provide crisis intervention and emotional support.</li>
<li>Collaborate with physicians and clinical staff to coordinate treatment plans.</li>
<li>Serve as liaison between patients, families, and the care team.</li>
<li>Assist with insurance navigation and community resources.</li>
<li>Maintain accurate and Timely EMR documentation in compliance with HIPAA.</li>
</ul><div><br></div><div><strong><u>Required Qualifications</u></strong></div><ul>
<li>Master’s degree in Social Work (MSW) required.</li>
<li>Active Illinois LCSW license required.</li>
<li>Minimum of 2 years of clinical experience preferred.</li>
<li>Experience in psychotherapy and crisis intervention.</li>
<li>EMR documentation proficiency (eClinicalWorks).</li>
<li>Strong communication and teamwork skills.<br><br>

</ul><div><strong><u>Preferred Qualifications</u></strong></div><ul>
<li>Hospice or palliative care experience.</li>
<li>Experience in chronic pain or integrated medical settings.</li>
<li>Experience monitoring patients on complex medication regimens.</li>
<li>Training or experience in CBT, ACT, or mindfulness-based therapy.</li>
<li>Experience working within an interdisciplinary healthcare team. </li>
