The Ambulatory Care Manager II is a master's prepared social worker that utilizes professional medical social work skills or Respiratory Care Practitioner to support the care team and patient population by facilitating the assessment of patients and families requiring psychosocial assessment, legal intervention, counseling support or respiratory care management, assessing, facilitating, planning, and advocating health needs on an individual basis. This position establishes, documents and implements treatment plans in collaboration with care team members. The Ambulatory Care Manager II will meet established organizational goals as outlined in "Franciscan Values and Service Standards".

WHAT YOU CAN EXPECT

  • Acts as a liaison and coordinates with a hospital multidisciplinary team, post-discharge providers and patient/family with the transaction of care.

  • Complies with all accrediting bodies, governmental agencies, and third-party payer requirements related to discharge planning and mandated reporting.

  • Maintains accurate, concise and timely documentation in EPIC and DOC flowsheets for discharge planning and payer authorization.

  • Provides clinical coordination of care, including chronic care conditions.

  • Reviews admissions, level of care, and continued stay utilizing nationally recognized criteria with respect to medical necessity, appropriateness of care and quality concerns.

  • Provides initial and continued stay reviews within the required period to the payers to justify hospitalization; identifies and documents avoidable days and reports quality concerns.

  • Works with quality department to improve the quality of care in acute and ambulatory settings.

QUALIFICATIONS

  • Master's Degree- Social Work- Required

  • Licensed Social Worker (LSW)- State Licensing Board- Indiana & Illinois- Required

  • Basic Life Support Program (BLS) American Heart Association- Required

  • 3 Years- Patient Care Experience- Required

  • 3 Years- Social Work Experience- Required

  • 2 Years- Case Management or Care Coordinator- Preferred
