Position Summary:  

The Patient Health Information Coordinator – PACE External Records, is responsible for obtaining, tracking, and maintaining outside medical records for PACE participants to ensure a complete, accurate, and timely health record. This role supports interdisciplinary care planning, regulatory compliance, and continuity of care by coordinating the receipt of records from hospitals, providers, facilities, and other external entities.  

Position Qualifications:  

  • High School diploma or GED required.
  • 1-3 years of experience in healthcare administration, medical records, health information management, hospice, or a related healthcare setting preferred.
  • Experience requesting External Medical Records preferred.  
  • Experience with electronic medical record (EMR) systems and electronic documentation platforms preferred.
  • Occasional travel to PACE sites is required for on-site meetings, audits, surveys, and related activities.
