About this role

As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. Most care services are provided at the PACE Center, including transportation to and from the Center. Be on the forefront of holistic care for our Kentucky community by joining our PACE program team!

Schedule: Monday-Friday 8:00am – 4:30pm 

Location: Frankfort and surrounding area

Position Summary:
The PACE Transportation Driver is responsible for the direct transportation of PACE participants to and from their destinations, including home, the PACE Center, medical providers, etc. Position requires constant interaction with PACE participants, caregivers, community agencies, medical providers and PACE program staff and leadership.

Position Qualifications:
· High school diploma or general education degree (GED) required.
· Must be at least 18 years of age and have a current Kentucky driver’s license, with a minimum of two years’ driving experience. 
· At least one year of work experience in transporting passengers.
· Minimum of one year’s experience transporting individuals with special mobility needs or must receive appropriate training upon hire.
· Minimum of one year’s experience working with a frail or elderly population or must receive appropriate training upon hire.
· Possesses and maintains current CPR certification.
· Food handling permit, to be completed upon hire.
· Proficient reading and writing comprehension skills.
· No positive drug or alcohol test including, pre-employment, random, post-accident, or reasonable suspicion.
· No more than 2 moving traffic violations during the previous 36 months. No driving under the influence (DUI) [Drug or Alcohol] conviction during the previous 5 years in a commercial or personal motor vehicle.
· Communicative, pleasant and courteous. Must be able to interact effectively with PACE participants, caregivers, IDT and PACE program staff.
· Able to keep accurate records; well organized.
· Entry level computer skills needed.
· Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
· Must be a licensed driver.
