About this role

As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. Schedule: Monday-Friday 8am-4:30pm, with occasional on-call

Position Summary:  

The PACE APRN exemplifies and promotes advanced nursing practice. Provides direct participant care and evaluates participant outcomes. Sets and evaluates practice standards. Provides consultation to nurses. Maintains own professional knowledge and skills as an expert and role model in the PACE Center’s advanced nursing practice. Works in close collaboration with Bluegrass Care Navigator’s Chief Medical Officer and the PACE Medical Director as well as community referral sources and medical specialists to establish the individualized participant centered plan of care (POC). Assists in providing a plan of care for the participant/family/caregiver as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, federal and state guidelines, and organizational policies and procedures, incorporating participant and caregiver input and requests as deemed appropriate. 


Providers of Bluegrass PACE care deliver the full range of ambulatory primary care services, including diagnosis and treatment of acute and chronic illness in geriatric age groups, preventative care, minor surgical procedures, and occasional medical emergencies. The providers are responsible for seeing their scheduled participants as well as walk-ins and add-ons and provide coverage during the operating hours of the Center. Provide telephone consultation as needed, perform administrative tasks associated with the provision of medical care, participate in quality improvement activities and provide community service. Medical providers are expected to manage all participants and conditions within the scope of their training and approved privileges.

Requirements:

  • Is a graduate of a Master’s Degree in a Nursing program. APRN certification as Family or Adult Nurse Practitioner.
  • In-depth, current knowledge in area of specialty and the ability to translate professional and organizational goals into effective practice.
  • Is currently licensed as an APRN through the State Board of Nursing.
  • At least three (3) years general nursing experience is required with palliative care, home health, long-term care, urgent care, or primary care experience. I.V. skills preferred.
  • Must have at least one year of experience working with the frail and elderly.
  • Food handling permit required, to be completed upon hire.
  • Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles.
  • Committed to the research process and application to practice.
  • Ability to organize resources appropriately.
  • Strong interpersonal skills, communication, collaboration, and conflict resolution.
  • Knowledge of systems and the change process.
  • Ability to manage multiple priorities simultaneously and effectively handles the emotional stress of the workload.
  • Complies with accepted professional standards and principles.
  • Understands Bluegrass Care Navigators (BCN) philosophy, principles of PACE care delivery system.
  • Possesses and maintains current CPR certification.
  • Have satisfactory references from nursing school, previous (or current) employers, and/or professional peers.
  • Has excellent observation, good nursing/medical judgment, and communication skills. Ability to provide good written and electronic documentation in a timely manner.
  • Is self-directed with the ability to work with little supervision.
  • Is flexible and cooperative in fulfilling role obligation and has ability to work within an interdisciplinary team.
  • Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
  • Must be a licensed driver with an automobile in good working order, that is insured in accordance with state and/or organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily injury coverage.
