About this role

As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. Schedule: Monday-Friday 8:00am-4:30pm             

               

Position Summary:

The PACE Clinical Manager provides leadership and oversight of all clinical operations within the PACE program, ensuring the consistent delivery of high-quality, person-centered care across the PACE Center, participants’ homes, and contracted provider settings. This role supports the PACE model of care by promoting participant independence, health, and well-being through coordinated, evidence-based clinical services.

Working closely with the Interdisciplinary Team (IDT) and Bluegrass PACE leadership, the PACE Clinical Manager ensures integration of clinical services, maintains regulatory compliance, and drives quality and performance outcomes. Key responsibilities include managing daily clinical operations, supervision and development of clinical staff, leadership of quality improvement and infection control initiatives, and fostering a culture of clinical excellence and continuous improvement within the nursing team.

 

Position Qualifications:

  • Graduate of an accredited school of registered nursing.
  • Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or participating COMPACT state Board of Nursing.
  • Associate’s degree in nursing required; bachelor’s degree preferred.
  • Two years of RN clinical experience required.
  • Two years of supervisory experience required.
  • One year’s experience working with frail or elderly populations within the last three years, or willingness to complete PACE-specific training prior to independent patient care.
  • Experience with Electronic Health Records (EHRs).
  • BLS certification required.
  • Food handling permit (to be completed upon hire).
  • Self-directed with the ability to work with little supervision.
  • Flexible and cooperative in fulfilling role obligations; able to work effectively within an interdisciplinary team.
  • Must be a licensed driver.
