About this role

As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. Most care services are provided at the PACE Center, including transportation to and from the Center. Be on the forefront of holistic care for our Kentucky community by joining our PACE program team!

Schedule: Monday-Friday 8am-4:30pm

Position Summary:
The PACE Certified Nursing Assistant provides personal care and related services in the PACE Center, home, hospital, nursing home, or ALF, under the direction, instruction and supervision of a Registered Nurse, PACE Center Director, or Home Care Manager. This position may require other duties and responsibilities as follows:

Participant Transportation:

  • Transport PACE participants to varied destinations timely, safely, and door-to-door.
  • Confirm that the PACE transportation vehicles are in proper working order, clean and safe prior to driving each day.
  • Assist passengers on and off the vehicle in a safe and courteous manner.
  • Follow established schedules and routes, with the flexibility to meet unanticipated changes, determine the safest and most efficient pick-up and delivery routes.

Requirements:

  • Is at least 18 years of age and has a High school diploma or general education degree (GED).
  • Is currently a State Registered Nurse Aide through the State Board of Nursing.
  • Proof of completion of recognized CNA training program.
  • Must have at least one year of experience working with the frail and elderly.
  • Food handling permit required, to be completed upon hire.
  • Possesses and maintains current CPR certification.
  • Is self-directed with the ability to work with little direct supervision.
  • Demonstrated knowledge and skills necessary to provide care to and communicate with the adult population.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write routine reports and correspondence.
  • Ability to work collaboratively with PACE staff and members of the Interdisciplinary Team.
  • Ability to speak effectively before groups of participants or employees.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized solutions.
  • Ability to adapt to participant status and needs as directed.
  • Ability to accommodate schedule changes daily due to staffing, participant condition and needs, and new participants.
  • Ability to follow emergency and evacuation procedures.
  • Must be a licensed driver with an automobile in good working order, that is insured in accordance with state and/or organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily injury coverage.
  • Demonstrates tact, patience, and good personal hygiene.
  • Employees will be expected to comply with Bluegrass Care Navigators Employee Health, infectious disease, and vaccination policies.
