About this role

Job Summary:
A licensed professional pharmacist employed by Erlanger Medical Center is responsible for providing and embracing pharmaceutical care for all patients. This is accomplished through the provision of appropriate drug therapy and clinical pharmaceutical services by reviewing patient orders and charts and by interacting with other appropriate healthcare providers and patients during physician rounds and educational opportunities.

Education:
Required:
Must be licensed or eligible for licensure in Tennessee and meet the following criteria:
Doctorate in Pharmacy (PharmD) or advanced training (3-5 years hospital experience, completion of a pharmacy practice residency, fellowship, and/or board certification).

Preferred: Completion of a pharmacy practice
residency and/or BCPS certification.

Experience:
Required:
Knowledge of general pharmacy practice and pharmaceutical care with emphasis on hospital practice; knowledge of organizational functions, policies and regulation; knowledge of current trends and new developments; knowledge of, or ability to learn, principles and practices applicable to those areas requiring special education.
Preferred: 3 years of appropriate clinical experience.

Required:
Current and Valid license to practice pharmacy in the State of Tennessee, or be eligible to be licensed as a Pharmacist by reciprocation or examination.

Preferred: Board Certification

Department Position Summary:
Provide Pharmacy review of medication orders prior to administration.
Work collaboratively with other clinicians and health-care providers to implement and maintain innovative disease management programs and clinical pharmacy services.
Participate in the development of medication use management programs within Erlanger Health System including clinical guidelines, critical pathways, disease management and drug use programs.
Participate in providing didactic and experiential training in Clinical Pharmacy for pharmacy students and clinical pharmacy residents.
Actively participate in clinical research projects resulting in presentation and/or publications.
Participate as adjunct faculty in accredited schools of pharmacy in the clinical and didactic training of pharmacy students.
Participate in the professional development and competency of ALL pharmacy staff while functioning as a role model and as an educational resource.
Assist in the development, implementation, and evaluation of residency programs in the area of Pharmaceutical Care and Clinical Pharmacology.
Keeps informed of all local, state and federal laws covering the storage, handling and dispensing of drugs and for interpreting each prescription order to determine that it meets all legal requirements.
Keeps informed of the action, side effects and proper usage of all new drugs as they are made commercially available, as well as all investigational drugs being studied at this institution.
Maintains awareness of contemporary trends in the profession through the professional literature and regular attendance at professional organization meetings, institutes and seminars as possible.
Participates collegially in the development of new programs, services and practices and in the education activities of the department.
Provides patient specific medication use teaching for discharge medications when appropriate.
Focus on cost avoidance and cost savings throughout the department and system.
Facilitate proper information transfer amongst clinical and pharmacy staff.
Take initiative to help and provide pharmacy services wherever needed.
Perform other related duties as required
Change work conditions based on volume and acuity level of patient population.
Daily use of computer/order entry/answering phones.

*A Pharmacist working at Erlanger Health System is required to have the ability to practice pharmaceutical care in a variety of settings and situations, possess sound judgment and organizational ability, and have above average written oral communication skills and interpersonal relationship skills.
