Lead Case Manager – CMA Medical Day Care Services Job Description

Responsibilities:

The Lead Case Manager provides support, mentorship, and quality assurance for the Medical Day Care Services Waiver (MDCSW) Case Management Program. This position ensures that case managers adhere to MDH standards, program protocols, and person-centered practices across their assigned regional cluster. The Lead Case Manager supports documentation accuracy, timeliness, compliance, and participant-centered care while helping maintain consistent service delivery across cases and counties.

Job Summary:

The Lead Case Manager is responsible for providing support to a team of Case Managers. This position plays a critical role in ensuring high-quality service delivery, conflict-free case management practices, and compliance with all MDCSW and LTSS Maryland requirements.

Essential functions include:

  • Reviewing and approving Plans of Service (POS) for quality, accuracy, completeness, and MDH compliance prior to submission.
  • Providing consultation on complex cases, safety concerns, SDOH barriers, and person-centered planning needs.
  • Conducting weekly case reviews to ensure timeliness of CSQs, PCPs, POS submissions, 180-day visits, and ongoing monitoring contacts.
  • Ensuring documentation accuracy in LTSS Maryland and providing corrective coaching when documentation does not meet standards.
  • Coordinating case coverage during staff absences to prevent service gaps and ensure regulatory timelines continue to be met.
  • Assisting Case Managers in completing in-person assessments, PCP development, conflict-free provider choice processes, RE reporting, and participant education requirements.
  • Collaborating with Adult Medical Day Care centers, Local Health Departments, MAP, and community providers to support service coordination and crisis planning.
  • Supporting the Program Director with training new staff, identifying learning needs, and delivering refresher training on documentation, engagement, compliance, and workflows.
  • Monitoring adherence to staffing ratios and caseload distribution, ensuring equitable and manageable caseloads across regions.
  • Participating in QA/QI activities, including chart audits, documentation reviews, and continuous improvement initiatives.
  • Serving as a regional point of contact for operational issues, provider communication, and participant escalations.

The Lead Case Manager may carry a small caseload if required due to program volume but primarily focuses on oversight, coaching, and quality assurance.

Lead Case Managers provide support across counties. The service area includes the following:

  • Baltimore City 
  • Baltimore County
  • Anne Arundel County
  • Harford County
  • PG County

Qualifications:

  • Energetic, supportive, and committed to participant-centered care.
  • Strong documentation and organizational skills with the ability to manage multiple priorities.
  • Excellent communication skills and the ability to mentor staff effectively.
  • Demonstrated experience providing case management or service coordination in aging services, behavioral health, long-term care, or medical day care settings.
  • Minimum of 2–3 years of case management experience required; supervisory or lead experience preferred.
  • Knowledge of COMAR 10.09.61, MDH Case Management requirements, and LTSS Maryland is strongly preferred.
  • Ability to conduct in-person assessments and support field-based activities across multiple counties.
  • Ability to interpret MDH regulations, implement program protocols, and support staff in meeting compliance expectations.
  • Ability to travel as needed.
  • Bachelor’s degree in Social Work, Psychology, Human Services, Public Health, or related field required.
  • Must pass Maryland CJIS background clearance and comply with Conflict-Free Case Management standards.

We will address an individual's goals in a conducive environment, identify areas of need and promote maximum independence and integration into the community through an array of rehabilitative services. PRP is an add-on service to primary mental health treatment provided by an individual's therapist or psychiatrist.
