Job Description

Elwyn Adult Behavioral Health Services is seeking a Licensed Practical Nurse (LPN).to assume responsibility for nursing care to all group home residential programs and supported apartments programs for young adults through geriatric individuals with chronic mental illness. The LPN provides generalized and specialized care; maintains appropriate records, functions as a member of an interdisciplinary team and performs related work as required; oversees the maintenance of medical records and compliance with all state regulations and CARF; and exercises independent nursing judgment.
 
ESSENTIAL FUNCTIONS:
  • Administers specified medication and takes vital signs as indicated and documents in consumer charts.
  • Transports individuals to medical appointments.
  • Transportation of prescribed medication as needed.
  • Reviews referral packets and assesses referrals for medical issues.
  • Provides nursing health evaluation information including assessment (if not provided), diagnosis, intervention and treatment planning information to other members of the mental health team.
  • Conducts emergency consumer assessments and refers to inpatient treatment as indicated.
  • Participates in treatment planning by: documenting use of clinical modality, completing assessment assignments within time requirements specified by procedures; scheduling/attending treatment planning meetings with staff and other individuals specified in procedures; utilizing assessment information, diagnostic results and staff participation in the development of treatment plans; and completing assessment, treatment plan and review documents following time and other technical specifications as indicated.
  • Reviews consumer records to ensure that medical goals are being addressed and notes recommendations for change and new clinical approaches, as indicated.
  • Ensures that all staff follows medication documentation as required by state specific medication guidelines.
  • Works directly in medical treatment relationships with consumers, individually or in groups, in consultation with the Psychiatrist and other physicians as indicated. Observes consumers and reports reactions to medication or treatment to medical personnel in charge.

  • Shows consideration and flexibility to support scheduled and emergent needs that occur within the system.
  • Takes active part in formal supervision, at least once monthly.
  • Participates in team meetings as required.
QUALIFICATIONS, EDUCATION AND EXPERIENCE:
  • LPN required, RN preferred
  • Non-profit human service experience highly preferred
  • Demonstrated ability to work effectively as part of a team
  • Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities
  • Demonstrated strong attention to detail
  • Demonstrated strong time management and organizational skills
  • Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions
  • Must be able to perform CPR and crisis intervention using agency-trained protocols
  • Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication
  • Demonstrated (basic, intermediate, advanced) experience with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint; Access, Publisher and report-writer experience preferred
  • (Experience using/knowledge of) Electronic Health Record (EHR) / Electronic Medical Record (EMR)
  • Must possess a current, valid driver's license in state of residence, have a minimum of two (2) years of driving experience within the United States, and have an acceptable driving record
