Position: Registered Nurse

Location: This position will cover the Adult Day Programs for Glendora, Bellflower, Norwalk, Carson, and Cerritos .

Hours: Part-time - Monday-Friday - 28 hours per week that will be between the hours of 8am to 4pm. Scheduled to be determined

Additional Information: Typically will cover one site per day but requires flexibility as needed. We pay for mileage. The position requires reliable transportation and a clean DMV record.

Hourly Pay Rate: starting rate $41.83 - $52.40 D.O.E.

OVERVIEW OF POSITION: Performs physical exams, health histories, health promotion, counseling and education, within the scope of the residential program assigned, allowing client(s) to move in. Administers medication(s), wound care, and numerous other personalized interventions, and sets up annual care plan for client(s). Interprets client(s) information and makes critical decisions about needed actions. Coordinates care in collaboration with a wide array of healthcare professionals. May direct and supervise the care delivered by other healthcare personnel such as LVNs. Conducts research in support of improved practice and client outcomes. Trains Direct Service Associates on health issues related to participant(s).

ESSENTIAL FUNCTION:

  • Conducts initial health assessment. Creates and maintains restricted and non-restricted health care plans annually or as needed. Conducts training of all staff on restricted and non-restricted health care plans annually or as needed. Monitors general health of residents with weekly assessments, including reviewing and monitoring of weight, medical appointments, labs, specialists, and related matters.
  • Delivers nursing care to residents within the scope of the residential program. Examines, evaluates and reports findings regarding residents’ well-being, including reported illnesses, injuries and/or medical device fixes. Provides recommendations for resolution to responsible individual/staff. Assists with emergency responses and remains on-call for illness and/or injuries and serves as first point of contact for associates. Reports all incidences to Residential Administrator.
  • Evaluates and conducts associate training within realm and expertise as needed. Ensures Medication Review procedures are followed by Direct Service Associates and LVNs where appropriate. Provides Medication Training to all new associates and provides on-going training/retraining for medication and universal precautions. May direct and supervise the care delivered by LVNs.
  • Maintains administrative documentation including annual assessments. Creates discharge summary plans as residents are released from hospital. Attends Annual ISP (Individual Service Plan) for the Regional Center and related meetings as needed.
  • Observes and reports health and/or safety issues to Residential Administrator or designee. Assists Residential Administrator in monitoring and disposing of expired medications on a monthly basis.
  • Perform other duties as assigned.

EDUCATION:

A valid Licensed Nurse Certification/Degree is required.| Must have and maintain a valid CPR and First Aid Certification.

EXPERIENCE:

  • Minimum 3-5 years of general Registered Nurse experience required.
  • 1+ years of experience working with medical fragile adults with disabilities in a residential home as a Registered Nurse preferred.

KNOWLEDGE, SKILLS, ABILITIES:

  • Ability to read, interpret and implement policies, procedures and regulations.
  • Ability to communicate effectively through oral and written communications with all levels of associates and the general public.
  • Ability to exercise discretion and maintain a high level of confidentiality.
  • Must maintain a high degree of professionalism and positivity in all interactions.
  • Ability to pass a post-offer physical examination and a TB test.
  • Ability to obtain and maintain a criminal record/fingerprint clearance from the Department of Justice and Federal Bureau of Investigation per Easterseals Southern California and/or program requirements.
  • Ability to travel locally with reliable transportation. Must maintain driving record in compliance with Transportation Safety Standards; maintain auto insurance and vehicle registration. Appropriate vehicle for transportation of program participants if program requires. Possess and maintain a valid California driver’s license.
  • Repetitive lifting of up to 50 pounds; standing, bending, reaching, stooping, squatting, kneeling, grasping, and/or twisting at the waist for extended periods of time.
  • Ability to pass all drug testing required by ESSC.
  • Ability to remain on call (24/7), and available during weekends and holidays.

Travel: Requires up to 30% local travel within assigned region
Environmental Exposure: Regular exposure to unpleasant or hazardous working conditions (noise, heat, dust, bodily fluids, etc.) more than 50% of work time.
