Patient Care Coordination Intern - Temporary Assignment
As a Patient Care Coordination Intern, you will work alongside healthcare providers and administrative staff to improve patient outcomes and streamline care processes. Your role will focus on applying health informatics and public health principles to enhance communication, coordination, and data management within clinical operations.
Core Responsibilities
Coordinate patient follow-up and care transitions across departments and community partners.
Assist in data collection, tracking, and integration through electronic health record (EHR) systems.
Support patient engagement initiatives that promote health literacy and equitable access to care.
Participate in program evaluation, quality improvement, and health compliance projects.
Collaborate with interdisciplinary teams to identify care barriers and develop data-driven solutions.
The compensation for this role includes a base pay range of $18-22, with the actual pay determined by factors such as skills, experience, education, certifications, geographic location, and internal equity. Additional compensation may be available through shift differentials, bonuses, and other incentives. Base pay is only a portion of the total rewards package.
