As a Life Enrichment Director, you will

• assess, document, and maintain patient/resident information related to care plans, MDS, participation records, progress notes, and reviews.

• complete initial activity assessments for new admissions, evaluating physical and cognitive abilities for participation in programs.

• participate in care plan conferences, clinical meetings, and interdisciplinary team meetings as required.

• plan, lead, and assist with group activities and patient/resident committee meetings; maintain required documentation and monthly meeting minutes.

• monitor, evaluate, and improve Life Enrichment programs to ensure quality, effectiveness, and resident engagement.

• coordinate with other departments and volunteers to deliver ongoing, high-quality programs that meet patient/resident needs.

• maintain confidentiality and protect patient/resident rights in accordance with organizational and regulatory standards.

• respond appropriately to emergency situations and changing resident needs.

• communicate effectively with patients/residents, families, staff, volunteers,

Preferred Qualifications:

• must have valid driver's license
• experience in Life Enrichment and / or working in an assisted living residence or skilled nursing center
• experience as a CNA or resident assistant is preferred but not required
• experience with memory care residents
