We offer an excellent salary and benefits package to include employer paid health insurance premiums for all full-time employees, PTO and paid company holidays, 403b retirement, EAP, and year-round employee wellness and appreciation initiatives.       POSITION SUMMARY: This is a professional position providing case management services to clients involved with the Housing First Division.  Targeted Prevention provides intensive case management services for families with dependent children who are not homeless and are housed, but are at-risk of becoming street homeless due to eviction or the loss of housing. The Targeted Prevention Specialist – Targeted Prevention will ensure that case management services focus on providing case management interventions that increase skills related to managing mental health and substance use issues as well as skills that increase housing stability.  The Targeted Prevention Specialist – Targeted Prevention will utilize natural supports, community supports, and input and resources from other community partners or professional agencies. The Targeted Prevention Specialist – Targeted Prevention ensures services are delivered using standards of trauma informed care principles, and that plans and clinical interventions address issues regarding how trauma impacts family functioning, and that plans and interventions use trauma resolution skills. The Targeted Prevention Specialist – Targeted Prevention ensures that services are Culturally & Linguistically Competent and are tailored to the unique culture of the child and family. The Targeted Prevention Specialist – Targeted Prevention will ensure that case management services occur in the community at a place convenient and comfortable for the youth and family. This is a field position.  The Targeted Prevention Specialist – Targeted Prevention is responsible for ensuring an annual client facing client care number of 1410 hours annually.  Achieving the minimum required weekly, monthly and annual client facing client care hours is an essential function of this position. Directions for Living is an essential services provider. All Directions for Living employees must be willing and able to provide vital support and essential services to the people that we serve in the event of an emergency, pandemic or other disaster situation. QualificationsEDUCATION EXPERIENCE/REQUIREMENTS: 1.Bachelor's degree in Psychology, Social Work, criminal justice or related field required, with a minimum of three years of previous experience in street outreach, crisis intervention, case management and supportive interventions to those who are living street homeless. 2. Must become certified as a Wraparound Facilitator in high fidelity Wraparound within 3 months of employment. Preference will be given to individuals certified as a Wraparound Facilitator in high fidelity Wraparound.  3. Must be knowledgeable of community resources available to individuals living street homeless. 4. Must be computer literate and be proficient in Outlook, Excel spreadsheets, and Microsoft Word. Must be able to work proficiently in a Windows environment and be able to learn other software applications quickly.  Must be able to document in to two separate systems on the same day as service was delivered. 5. Must have reliable transportation and be able to use personal vehicle to transport clients if and when needed.  Must have and maintain clear driving record, must have valid Florida Driver License, maintain required automobile insurance coverage.
