Position: Acute Service Coordinator

Pay Range: $26.28 - $29.01 per hour, based upon education and related experience

Schedule: The normal workweek for this position is 37.5 hours M-F, however, the incumbent must be extremely flexible with their schedule in providing evening and weekend hours when necessary and/or to deal with a crisis or other unusual situation as needed.

Devereux/TCV is currently seeking a dedicated and responsible person to fill our Acute Service Coordinator position in Braddock, PA

 

 

Job Description:

 

We are looking for a person who believes in a strengths-based approach as the ASC will serve those experiencing homelessness and who have behavioral health challenges in connecting them to crucial services and supports during the transition period to housing. In addition, the ASC will assist individuals leaving inpatient psychiatric care in making the transition to outpatient services and community supports.

 

The ASC position is designed to:

  • Engage and develop relationships with the individual and his/her family in order to build lasting relationship.
  • Assess the individual and family strengths and needs in collaborative method through individual and/or collateral interviews and reviews of social and clinical information provided by other entities.
  • In close collaboration with the consumer, family members and other service providers, promote service planning efforts which result in developing, documenting and implementing a comprehensive service plan driven by the individual utilizing all the agreed upon strengths and needs.
  • Evaluate all services received by individuals who are served by the Service Coordination program.
  • Investigate new resources and communicate with providers of prospective resources as a liaison on behalf of the individual being served.
  • Convene and facilitate interdisciplinary service planning meetings or other related team meetings to ensure appropriateness and responsiveness of services in relation to individual and/or family needs.

Qualifications

EDUCATION & EXPERIENCE:

  • Bachelor's Degree in Social Work, Psychology, Criminal Justice, Counseling, Sociology, Education with a certificate in Special Education or Nursing with a Registered Nurse License.
  • Two (2) years of human service working experience with knowledge of mental illness and other human services.
  • Child Abuse History, Criminal Clearance (ACT 33/34) and FBI Clearances.
  • Valid PA driver’s license with three (3) year driving history.
