About this role

Overview

Plan, organize and implement nursing care and education of adult patients, and adult patients undergoing treatment with coumadin/warfarin.

Responsibilities

Clinical:

  • Coordinates patient care with the Primary Care Provider and other care providers, as appropriate.
  • Maintains established departmental policies and procedures, objectives, safety, and infection control standards.
  • Inform patients of PT results, dose of medication to take, and time frame for follow-up.
  • Recognizes patient problems and refers any concerns regarding medication regimen, bleeding, or other adverse physical conditions to the physician.
  • Assists patients arriving in the clinic, and takes vital signs as needed, referring patients to the physician’s office or ER, as indicated.
  • Performs prothrombin/INR blood tests and records appropriately.
  • Ensures patients are knowledgeable regarding their medication, providing additional education as needed.

Administrative & Clerical:

  • Assists in the development of nursing policies and procedures related to the care of patients receiving specialty services.
  • Maintains accurate records on each patient.
  • Assists in registering new patients, scheduling appointments and follow-ups on missed appointments, as necessary.
  • Prepares patient education
  • Prepares daily list of pending PTs
  • Completes and sends out patient status reports to referring providers.
  • Enters PT/INR results into computer system.
  • Assists lab with yearly correlation studies.

Clinic Maintenance:

  • Keeps patient care areas clean and neat.
  • Ensures availability of supplies and equipment
  • Checks control accuracy on testing device and records monthly.
  • Cleans and disposes of blood testing equipment.

Qualifications

  • Graduate of an accredited school of nursing.
  • 1-3 years general nursing experience in in a hospital/clinical setting.
  • Current state license as appropriate is required.
