Position Summary
The Outreach Coordinator Analyst supports quality initiatives by closing gaps in care, engaging providers and members, and serving as a subject matter expert for HEDIS® measures and performance improvement activities.

Key Responsibilities

• Serve as a subject matter expert for HEDIS® and quality improvement measures
• Conduct provider and member outreach to close gaps in care
• Facilitate meetings with providers and internal stakeholders
• Plan and execute outreach workplans using data and analytics
• Support medical record collection and abstraction
• Deliver education and presentations to internal and external partners
• Identify and implement process improvement opportunities
• Data analytics and presentation

Required Qualifications

• 3+ years of healthcare experience
• 1+ year of provider or member education experience
• HEDIS® and quality improvement knowledge
• Proficient knowledge of Microsoft applications excel, word and outlook.
• Strong written, verbal, and presentation skills
• Ability to work independently in a metric-driven environment

Preferred Qualifications

• Bachelor’s degree or equivalent healthcare experience
• Certified Health and or Wellness Coach
• Experience working with medical providers or office staff
• Knowledge of medical coding
• Managed care or health insurance experience

Education

• High school diploma or GED

The typical pay range for this role is:

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
