Evaluate and admit patients to hospice care, develop a plan of care, and provide care to the patient according to plan.
Provide end-of-life care and coordination at time of death and bereavement support following death.
Act as case manager making necessary adjustments in the patient's plan of care related to symptom changes and changes in care needs.
Act as liaison for patient/caregiver with MD, community health care partners and other hospice team members.
Complete all documentation related to patient contact, assessments, and IDT meetings according to policy.
Provide on-call coverage for both home care and hospice as assigned.
Make necessary home care visits as assigned.
Demonstrate and encourage team behavior and exceptional patient/guest experiences.
Uphold and promote patient safety and quality.
Education:
Associate's degree in nursing and currently licensed in the state of Alabama required.
Experience:
Minimal 2 years' experience in a hospital setting working in medsurg, critical care or ER. Previous home care or hospice experience preferred.
Additional Skills/Abilities:
Must exhibit strong critical thinking and organizational skills. Must be able to work independently and make independent decisions. Must be flexible in scheduling. Must be able to organize and prioritize a schedule to meet patient needs.
