Join our compassionate team as a Full-Time RN Case Manager!
Salary Range: $92,000 - $95,000 DOE.
We are seeking a dedicated and compassionate Full-Time Registered Nurse Case Manager to join our healthcare team. This role is crucial in coordinating patient care, ensuring effective discharge planning, and managing the overall health needs of patients across various settings, including homes, nursing facilities and assisted living scenarios. The ideal candidate will possess a strong background in patient assessment and care management, with experience in specialized areas such as geriatrics, long term care or home healthcare.
Duties
- Conduct comprehensive patient assessments to develop individualized care plans.
- Collaborate with interdisciplinary teams to coordinate patient care and facilitate effective communication among healthcare providers.
- Manage discharge planning processes, ensuring patients receive appropriate follow-up care and resources post-discharge.
- Monitor patient progress and adjust care plans as necessary to meet evolving health needs.
- Advocate for patients by providing education on their conditions, treatment options, and available resources.
- Maintain accurate documentation of patient interactions, care plans, and outcomes in compliance with healthcare regulations.
- Stay informed about best practices in managed care and apply them to improve patient outcomes.
Experience
- Current Registered Nurse (RN) license in the State of Missouri.
- Demonstrated ability in patient assessment and case management practices.
- Knowledge of discharge planning processes and managed care principles is highly desirable.
- Strong interpersonal skills with the ability to work collaboratively within a team environment.
- Excellent organizational skills and attention to detail in managing multiple cases simultaneously.
