We are seeking a compassionate and dynamic individual to join our team as a Care Coordinator for the unit. In this role, you will have the opportunity to coordinate care for our patients, making a meaningful difference in their lives and supporting them on their journey to recovery. Please note that all applicants must undergo a pre-employment drug screening, which includes testing for marijuana, as well as a comprehensive background check and TB Test. ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.  Responsible for the development of care planning and patient needs assessments.  Facilitates life skills groups in accordance with the CTP policies and procedures. Acts as liaison, establishing networks and collaboration with hospitals, community health centers, and state and human service agencies about patient services and needs.  Document admissions, care management needs, discharge, and discharge planning.  Administer case management assessments to identify patient needs and ensure the Addiction Severity Index (ASI) is completed at intake.  Organize a schedule of patient bed dates, transitioning levels of care, and consistent communication with the Medicaid Placement Coordinators.  Assists treatment team with patient intake, transfer, and discharge assessments.  Collaborating with the facility treatment team (medical, psychiatric, and SUD) to identify patients’ needs. From available resources, assist patients in meeting their basic needs (i.e., clothing, medical, judicial, and religious).  QualificationsQUALIFICATIONS: Education: An associate's or bachelor's degree in human services.  Obtain the CAT certification within 18 months of your hire date. Obtain a case management certification within 90 days of your hiring date. Experience: Minimum of two (2) years of experience working in the substance use and mental health field. Other: Must have a good understanding of substance misuse, mental health, and treatment delivery systems. Advanced computer skills required. CPR and First-Aid certification (the agency pays for both initial training and the renewal courses). Must be able to take vital signs, recognize possible medical problems, and refer to the medical staff when necessary. Valid driver's license and good driving record. Able to communicate effectively in a multidimensional treatment program, including medical, psychiatric, and substance misuse-related issues.
