This position is responsible for the coordination and implementation of the specialized activity focused program within the Memory Support area of Brandel Health and Rehab. This position shall provide direct day-to-day care and activities for the residents. Coordinates training/in-services of residents, family members, and unit personnel. The person in this position will adhere to facility policies and procedures and meet state and federal regulatory guidelines.

NATURE AND SCOPE:

This position reports directly to the Administrator - Health Care and is supervisory in nature, overseeing a team of inter-disciplinary members (approximately 8-12 FTE), as well as providing direct resident care, as needed.  This individual will also work closely with the Director of Nursing, Life Enrichment  Director/Supervisor, Social Service Director, and the Director of Food Service.  In performance of duties, the Memory Support Coordinator is entrusted with the following responsibilities:

Participates in the development, implementation, and maintenance of policies and procedures for the program.  Annually determines compliance or need for revision.  Works within parameters set by Administration and the Orchard Court Operations Manual and supports and respects the team philosophy.

Establishes and actively participates in therapeutic recreational activities that promote a holistic framework with outcome measurements involving cognitive, cultural, physical, and psychosocial components.

Participates in the development, implementation, and monitoring of assessment criteria for residents with diminished mental function assuring that they are receiving appropriate services.

Assists in the orientation and assessment of campus residents transferring to and from the unit.

Acts as a liaison with residents, families, physicians, staff, and other professionals.  Supports the residents’ families and significant others as needed. 

Directs, schedules, and implements the Life Enrichment programs within the unit on a daily flexible schedule.  Assists the Life Enrichment Therapist in contacting outside groups, entertainment, and intergenerational programs.  Cooperates in planning and implementing community outings.  Acts as a liaison within the Health Care facilities at large.

Confers with residents, families, Life Enrichment personnel, and unit staff to plan appropriate activities to promote interest and participation of residents in various programs.  Ensures that new residents’ needs and interests are addressed.

Observes on an ongoing basis resident conditions including cognitive, physical, and general well-being.  Reports observations to other personnel, as appropriate.  Attend daily stand-up meeting. 

Monitors on an ongoing basis the evaluation and effectiveness of programs and activities.  Works closely with Life Enrichment Therapist staff to ensure programs and activities are revised as necessary.  Keeps current with trends and developments in the field. 

Responsible for all unit records including Social Services’ MDS documentation, resident assessments and progress notes.  Participates in the Care Planning process.

Participates in the interviewing of candidates for Life Enrichment Therapist positions within the unit.

Educates new unit personnel in the specific philosophy and associated care techniques of the unit.  Provides orientation and training.  Directs professional and non-professional staff in their assignments/areas of responsibility.  Coaches, counsels, and motivates unit members on an on-going basis.

Participates in developing and conducting educational programs and in-services on dementia-related diseases as needed for all staff, volunteers, and family members. Ensures 4 hours of dementia training is provided to all new staff that work in the memory support area.  Ensures that 12 hours of dementia training is provided to all staff who work 50% or more of their time in the area. 

Reviews shift reports of all residents within the area on a daily basis to assure proper communication of information regarding resident activity/needs.

Ensures the instruction and assistance of residents with activities. Works with Life Enrichment staff to obtain films, slides, and cassettes for use in resident activities. Provides appropriate reading materials for residents. 

Monitors the use of equipment, supplies, and human resources of the unit for economic/efficient usage.  Maintains records of Life Enrichment expenses and submits them to the Life Enrichment Supervisor – Health Care. Participates in the budget process.

 In coordination with Life Enrichment Supervisor, acts as Volunteer Coordinator for Brandel Health and Rehab.

Communicates on a regular basis with the Administrator and appropriate departments regarding concerns, needs, and conditions within the facility.

Coordinates, processes, and distributes a calendar of events for the unit.  Works with Life Enrichment staff to ensure all activities are added to calendar in a timely manner.  

Participates in the healthcare facilities   QAPI Program. 

Assists in the transportation of residents in case of fire, disaster, or emergency evacuation according to procedure.

Performs other related duties as assigned.

QUALIFICATIONS:

This position requires a minimum of:

  • Associate's degree, Bachelor’s degree, preferred; or
  • be a registered nurse and have at least one (1) year of experience working with persons with Alzheimer’s disease and other dementia; or
  • A minimum of five (5) years experience working with persons with Alzheimer’s disease and other dementia; at least two (2) years of which are within a management/supervisory position. 
  • The individual shall have documented course work in dementia care and ability-centered care.

The ability to understand the needs of and show compassion to the dementia client.  An awareness/knowledge of state and federal regulations for skilled nursing facilities, preferred.

The individual must possess excellent communication skills (English, oral and written) for interaction with residents, family members, staff, and others.

Target Pay Rate:

$26.00/hour

Compensation Pay Range:

$19.13 - $23.33 per hour

Reasonable Pay Estimate

A reasonable estimate of the pay range for this position is $19.13 - $23.33 per hour. There are numerous factors taken into consideration in determining the actual offered rate of pay, including but not limited to: job-related qualifications, experience, skills, education, geographic location, and consideration of internal and external equity.
