We are seeking a Lead Care Coordinator to oversee treatment-floor operations and support clinical excellence across rehabilitation services. This role blends leadership, workflow oversight, and hands-on clinical coordination to ensure high-quality patient care, accurate documentation, and seamless interdisciplinary collaboration.


What You’ll Do

Clinical & Operational Leadership

  • Lead daily treatment-floor operations and maintain an active leadership presence.

  • Monitor patient flow and adjust assignments to support efficiency and quality care.

  • Reinforce therapy safety, provider directives, and clinical best practices.

  • Support Rehab Care Coordinators with provider-directed therapies and DME dispensing and training.

Team Development & Coaching

  • Participate in interviewing and selecting Rehab Care Coordinator candidates.

  • Train, onboard, and mentor new team members.

  • Provide ongoing coaching, feedback, and performance development.

  • Contribute to performance evaluations and growth plans.

Care Plan Compliance & Outcomes

  • Monitor care plan adherence and escalate concerns when needed.

  • Ensure provider expectations and therapy components are fully executed.

  • Support KPIs tied to visit adherence, order fulfillment, and care completion.

Documentation, Orders & Accuracy

  • Audit therapy documentation for accuracy, completeness, and timeliness.

  • Ensure provider-directed orders are clearly communicated and completed.

  • Support documentation workflows that reduce billing errors and delays.

Insurance & Authorization Oversight

  • Train team members to identify insurance and authorization barriers.

  • Coordinate with Administrative Care Coordinators on authorization needs.

  • Escalate eligibility or coverage concerns to leadership.

  • Ensure services are delivered only when coverage is confirmed to protect revenue integrity.

Staffing, Scheduling & Collaboration

  • Support Practice or Hub Managers with staffing and clinical flow decisions.

  • Identify workflow bottlenecks and recommend process improvements.

  • Collaborate closely with providers, front desk leadership, and administrative teams.

  • Support smooth transitions across medical, imaging, rehab, and chiropractic services.

  • Ability to multitask, prioritize, and remain composed in a fast-paced environment

Experience

  • Required: Minimum 1 year of experience as a Care Coordinator

  • Preferred: 2–3 years of clinical support experience with leadership responsibilities
