Community Healthcare of Texas has provided Hospice and Palliative Care Services since 1996.  Community Healthcare of Texas has cared for patients with serious and terminal illnesses throughout North Central Texas.  Providing compassionate care for those living with an illness while supporting those caring for a loved one is the mission of Community Healthcare of Texas.

Community Healthcare of Texas is currently recruiting for an Inpatient LVN. 


POSITION SUMMARY

Assist RN in the overall plan of care for the patient. 

ESSENTIAL FUNCTIONS 

  • Assess the patient's physical, psycho-social, and spiritual condition; contribute to the plan of care in collaboration with an RN and provide palliative nursing care. 
  • Provide emotional support for patients and families/caregivers while instructing in end-of-life care/issues. 
  • Assists RN in case management of total plan of care of patient/support persons by coordinating with team members, communicating with IDG, and supervising CNAs. 
  • Document nursing care and significant interactions with patient support persons and team members. 
  • Participate in follow-up with bereaved families with various types of contact.
  • Assist RN in the admission of a patient to Hospice House, including immediate crisis management and orientation. 
  • Assist RN in the discharge of patients and coordination of transfer, including instructing patients/caregivers in follow-up care. 
  • Assist RN in the management of multiple patients and their support persons simultaneously in crisis with acute symptoms, including pharmacology.  

POSITION QUALIFICATIONS

  • Licensed Vocation Nurse licensed by the State of Texas.
  • Must have emotional/spiritual maturity to deal with patients/support persons according to their crisis needs.
  • Able to work under the supervision of RN within guidelines/protocol.
  • Able to make appropriate decisions affecting patient care, consulting the RN in charge if beyond the scope of LVN practice. 
  • Able to manage multiple tasks regarding immediate patient care. 
  • Must be able to pass Hospice back screening examination prior to employment. 
  • 1 year of nursing experience is required.
  • Hospice/home health experience preferred.
