Community Healthcare of Texas has provided Hospice and Palliative Care Services since 1996.  Community Healthcare of Texas has cared for patients with serious and terminal illnesses throughout North Central Texas.  Providing compassionate care for those living with an illness while supporting those caring for a loved one is the mission of Community Healthcare of Texas.

Community Healthcare of Texas is currently recruiting for an Field LVN. 


POSITION SUMMARY

  • Assist the RN Case Manager in the plan of care for the patient.
  • Assists RN in case management of total plan of care of patient/support persons by coordinating with team members and communicating with IDG in accordance with organizational policies, procedures and regulations that define the scope of practice.
  • Complete, maintain and submit accurate and relevant clinical notes regarding the patient’s condition and care given.  Record pain/symptom management changes/outcomes as appropriate.
  • Administer medications and treatments as prescribed by the physician.
  • Document nursing care and significant interactions with patients, support persons, and team members.
  • Assess patient/family response to care and participate in follow-up with bereaved families with various types of contact.
  • Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis.
  • Participate in on-call rotation

POSITION QUALIFICATIONS

  • Licensed Vocational Nurse licensed by the State of Texas
  • Hold a valid driver’s license and have a dependable means of transportation
  • 1 year nursing, hospice, or oncology experience preferred
  • 1 year LVN experience in an acute care setting required.
  • Proficient skill using a computer is required, Microsoft office suite experience preferred.
