About this role

Description

The APN/NP is a specialty health care provider who provides medical and nursing services to individuals and families. The APN/NP is adept at diagnosing and managing acute and chronic illnesses, and emphasizes health promotion and disease prevention. Services include, but are not limited to, ordering, conducting and interpreting diagnostic and laboratory tests, prescribing medications, and teaching counseling individuals and families. Comprehensive health assessments with a broad scope of medical management for patients with acute and/or chronic health problems characterize the APN/NP’s practice. Ad

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Perform approved inpatient procedures, if trained and credentialed (including endotracheal intubation and central venous line placement); training will be provided as needed for procedures and support provided by supervising physician. 
  2. Provide first-line in-house coverage, escalating care to the supervising hospitalist as needed. 
  3. Independently manage acutely ill patients using ACLS protocols and evidence-based care pathways, with supervising physician guidance. 
  4. Lead and manage Code Blue and Rapid Response events in collaboration with, and with immediate access to, supervising physician for guidance and support  
  5. Demonstrates clinical expertise in the diagnosis and management of a wide range of health problems.
  6. Develops medical treatment plans based on scientific evidence-based practice and evaluates outcomes.
  7. Mentors staff/students in aspects of advanced nursing practice and patient care
  8. Conducts comprehensive appraisals of patient health status by taking health histories and performing physical examinations.
  9. Initiate and participate in admission histories and physicals, medical consultations in related subspecialty, and discharge planning.
  10. Order diagnostic tests and procedures. Establish a diagnosis based on the history, physical examination and diagnostic test results. Formulates differential diagnosis.
  11. Evaluate results of diagnostic testing of laboratory tests, EKG’s imaging studies, and medical procedures.
  12. Make appropriate consultations and referrals.
  13. Develop a treatment plan based on diagnosis and evidence-based practice that is mutually acceptable by the patient and family.
  14. Determine the effectiveness of the treatment plan with documentation of patient care outcomes.
  15. Reassess and modify the treatment plan to achieve optimal health and medical goals for the patient.
  16. Teaches and counsels families in medical conditions, treatment plans, preventive care and anticipated outcomes.
  17. Documents in the medical record with SOAP charting all care provided or procedures performed by the APN/NP. Include differential diagnosis in the documentation.

Requirements

Education and Experience

  • Master's degree in nursing
  • Three-Five years’ experience in a clinical role.
  • Experience in Staff education, and quality management preferred.
  • Experience working with teams as a member and leader.
  • Good customer relations and interpersonal skills.
  • Preferred computer skills include, /Microsoft Word, Excel and Power Point.
  • Current licensure by the Illinois State Board of Nursing
  • CPR certification per hospital requirements
