What You Will Do as a Hospice Nurse Practitioner

As a Hospice Nurse Practitioner, you will provide health care to patients and education to their families through the application of advanced knowledge and skills. In collaboration with the Hospice Medical Director, you will provide services related to health promotion, chronic disease management, and end of life care.

Some of your primary responsibilities will include:

  • Perform face-to-face encounters with the patient to determine continued eligibility for hospice care within time frame of regulatory guidelines.
  • Document clinical findings in detail at patient bedside as related to the hospice criteria and provide a clinical report to the hospice physician within 24 hours of visit.
  • Prepare and maintain accurate patient records and documentation to support interdisciplinary team and services provided.
  • Comply with legal, regulatory, and accreditation requirements.
  • Evaluate hospice patients for appropriate level of care.
  • Assist in development of hospice protocols, policies, and procedures as required.
  • Participate in QAPI activities as requested.
  • Participate in in-service education related to areas of practice as required.
  • Address and support cultural practices, as long as practices do not harm others.
  • Adhere to all organization, payroll, billing, and documentation policies and procedures.
  • Maintain knowledge of current federal, state, and accreditation standards and organization regulations concerning patient care.
  • Adhere to MSA policies and procedures.
  • Complete annual educational requirements.
  • Ensure compliance with state and federal regulations, company policies/procedures, and process maps in the performance of job responsibilities.
  • May provide on call support to staff.
  • Contribute to team effort by accomplishing related results as needed.

Qualifications

  • Valid Nurse Practitioner’s license
  • Master’s degree in nursing
  • Previous hospice and palliative care experience preferred.
  • Must have reliable transportation. This position involves daily home visits and weekly office meetings.
  • Valid driver’s license and company-required auto-liability insurance
  • Must be flexible and able to adapt to changing client status and needs, schedule changes as directed.
