Job DetailsJob Location: McKinney, TX 75069Position Type: Full TimeEducation Level: 4 Year DegreeSalary Range: $21.63 - $26.44 HourlyThis Access and Diversion Qualified Mental Health Professional (QMHP) Community Services (CS) reports directly to the Access and Diversion Supervisor within the behavioral health division. This position will serve individuals who present to LifePath Systems Diversion Center and perform face to face crisis assessments, crisis intervention, crisis follow up and relapse prevention services, intake assessments, recovery and safety planning, resource navigation, referrals to appropriate community services, and connection to outpatient behavioral health services.   This position connects individuals from the community to LifePath Systems services. Uses initiative and a passionate commitment to provide access to care to individuals in services as well as diverting from emergency rooms, hospitals, and jails. Access and Diversion responsibilities include ensuring individuals receive timely access to appropriate services, working with the entire LifePath Systems team and other community organizations to identify needs and reduce barriers to improve social determinants of health. Must be effective at troubleshooting and organizing support, interventions, and services to achieve outcomes. This position is crossed trained with the Crisis Center to perform assessment of crisis resolution and stabilization and linkage to services to reduce inpatient and law enforcement interventions. Other duties as assigned to meet program and individual needs.   This position exemplifies service excellence by providing an overview of LifePath Systems services and then connects to outpatient behavioral health services. Services will be provided with a trauma-informed and person-centered approach.   This position practices good stewardship of resources by ensuring all required training is completed prior to due date and center-issued equipment is maintained in good condition. The Access and Diversion QMHP-CS will link individuals to additional resources as needed to support their goals.      Integrity is necessary to ensure adherence to the Health and Human Services Commission, Texas Administrative Code, contracts, applicable grants, and Lifepath Systems policies and procedures. This position will work closely with fidelity managers and leadership to ensure that all gaps are reconciled.   This position builds positive relationships with local police agencies and mental health professionals in an effort to increase referrals from community stakeholders and partners. This position demonstrates professional conduct with Peers, Leadership and Support teams at all times. In addition, close working relationships and professional conduct while coordinating care with outside entities is required.   Working independently and collaboratively to maintain appropriate boundaries and foster better outcomes for individuals served and facilitating improvement in measurable ways, is central to LifePath Systems mission to serve. This position must have the ability to assess and de-escalate behavioral health crisis situations in a calm and respectful manner.   Specific Targets: Engagement with individuals served and exceptional customer service will be provided and documented in the electronic health record a minimum of half of time worked. At least 90% of service notes are signed within 2 business days of service. All crisis documentation is completed concurrently and signed by the end of shift. Assigned follow ups are completed at 24 hours, days 1-7, 30, 60, and 90-day intervals. Shift report is communicated at the end of each shift. Full chart reviews receive a score of 80% or higher following the Golden Thread. Provides person directed services to assist in navigating access to medical, social, vocational, financial, educational, and other individualized services with a focus on access to care and diversion from incarceration. Must understand criteria for LifePath Systems services and be able to explain to individuals served as well as community partners. Attends monthly team meeting.    Due to operating 24/7, employees may be required to work on center holidays if the holiday falls on their scheduled workday. In the event of unforeseen circumstances that require the use of Paid Time Off, employees are expected to notify their supervisor within 4 hours of their regularly scheduled work time.   If Bilingual and receiving stipend, employee will be available to translate for other staff as needed and will utilize approved language in the provision of services as needed. Qualifications Bachelor’s degree in psychology, social work, or related field from a four-year college or university. CCBHC knowledge and experience preferred. Experience in Texas Recovery and Resiliency (TRR) preferred. Knowledge of Trauma-Informed Care Practices required. Must have reliable transportation and a valid driver’s license.
