Join our Targeted Case Management (TCM) Program and help transform lives by supporting individuals who are chronically homeless or recently discharged from hospitals or correctional facilities.

As a Targeted Case Manager, you'll be a vital advocate and connector—empowering adults with behavioral health challenges to access housing, healthcare, and essential community resources. Through personalized, recovery-focused support, you’ll help individuals build stability and thrive.

What You’ll Do:

  • Complete community-based needs assessments for individuals on your caseload
  • Provide case management and support services aligned with each person's Individual Recovery Plan (IRP)
  • Facilitate care coordination and communication across service providers and support networks
  • Participate in interdisciplinary treatment team meetings to inform care strategies
  • Make consistent weekly outreach to all clients and maintain high-quality documentation
  • Advocate for clients and help them navigate housing, healthcare, legal systems, and more
  • Provide transportation for individuals to community appointments and integration events
  • Communicate clearly and ethically with agency staff, families, and community partners
  • Support family education and help leverage natural supports in the community

 

Minimum Qualifications:

  • Bachelor’s degree in Social Work, Psychology, Human Services, or a related field
  • Proficient with technology tools, including electronic medical records, smartphones, and telehealth platforms
  • Strong care coordination skills to connect clients with internal and external services
  • Valid Georgia Driver’s License and ability to pass a defensive driving course

 

Preferred Experience:

  • 1+ year of behavioral health or human services experience
  • 2+ years in residential services or working with vulnerable adult populations
  • Familiarity with trauma-informed care and recovery-oriented approaches

 

Working Conditions:

  • Primarily community-based field work, with some in-office documentation and meetings
  • Service delivery through both in-person and telehealth
  • Occasional evening/weekend work for community events or special circumstances
  • Use of agency vehicle for transporting individuals
  • Must complete all agency-required trainings and certifications
