About the Role:
The Hospice RN Clinical Manager plays a pivotal role in overseeing the clinical operations of hospice care services to ensure the delivery of compassionate, high-quality end-of-life care to patients and their families. This position is responsible for leading and mentoring a team of hospice nurses and clinical staff, fostering a supportive environment that promotes professional development and adherence to best practices. The manager collaborates closely with interdisciplinary teams, including physicians, social workers, and chaplains, to coordinate comprehensive care plans tailored to each patient's unique needs. Additionally, the role involves monitoring clinical outcomes, ensuring compliance with regulatory standards, and implementing quality improvement initiatives to enhance patient care. Ultimately, the Hospice RN Clinical Manager serves as a clinical leader and advocate, driving excellence in hospice care across the organization within the United States healthcare framework.
Minimum Qualifications:
As a Hospice Clinical Manager, you will:
Direct and coordinate clinical departments; assume responsibility for continuity, quality, and safety of services delivered in compliance with state and federal regulations. (Conditions of Participation).
Promote the philosophy and administrative policies of the Agency, in an effort to ensure quality of patient care.
Participate in activities relevant to professional services including development of qualifications and assignment of agency personnel.
Supervise and provide direction to subordinates, in an effort to ensure quality, compliance with the Plan of Care, assessment and reassessment of patients’ needs, and continuity of services by appropriate health care personnel.
Monitor the QAPI Program and assure appropriate corrective measures are performed.
Promote compliance with all fiscal intermediary and/or other third party payors, through education, coaching, and other assistance as necessary.
Participate as a member of the Interdisciplinary Group (IDG), assists in development of the IDG plan, and participates in QAPI activities.
Monitor systems and identify problem areas to the Administrator both verbally and through written reports.
Meet mandatory continuing education requirements of the agency and licensing board.
Promote and educate regarding the concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.
Use effective interpersonal relations and communication skills; facilitate the use of these skills by other team members to achieve desirable outcomes.

Valid Auto Insurance
CPR Certification
At least 2 years of Hospice management experience preferred
HomeCare HomeBase experience preferred
