  • Under the supervision of licensed mental health multidisciplinary team staff, provides consultative support to the multidisciplinary patient team members (ex. medical providers, advanced practice providers, social workers, nurses, etc.) regarding psychiatric/psychosocial concerns and behavior change, various behavioral health topics, and evidenced-based behavioral approaches for treatment.

  • Under supervision of licensed team staff, supports ongoing evaluation protocols for patients with positive behavioral risk health screens to determine need for further assessment and strategies to optimize safety, experience, and clinical outcomes. Participates in the creation of an integrated, individualized, evidence-based plan of care for patients and families with identified clinical need while in the hospital, ED, ambulatory settings.

  • Collaborates with the licensed clinical staff on behavioral planning, care, and problem-solving for longer-term and higher acuity patients, procedures, and other challenging situations.

  • Works as part of larger team to increase the comfort and knowledge of care providers and other staff in safety and of patients’ mental and behavioral health needs and the mental and behavioral health systems of care; regularly communicates and updates the clinical team on topics of interest related to behavioral health management through various modes of communication.

  • Facilitates the involvement of patients/families in care decisions, goal setting, and discharge planning as needed. Interfaces and provides input into community partners to coordinate continuity of care and facilitate referrals and transitions as appropriate.

  • Develops and contributes to educational health literate materials for patients, providers, and clinical staff that meet the cultural needs of the patients, as needed.


Requirements

  • High School graduate or Certificate of General Educational Development (GED) or High School Equivalency Diploma (HSED) required

  • Obtain formal training in CW Trauma Informed Care Initiatives required; Marcus Crisis Prevention Program (MCPP) Trainer - Marcus Autism Center Crisis Prevention Program within 120 Days required

  • Bachelor's Degree in Social Work, Psychology or related field preferred

  • 2+ years experience in behavioral health IOP, PHP, inpatient or residential treatment required

  • Experience in a pediatric primary care or other pediatric medical setting preferred

  • Experience working with children and families, diverse ethnic, cultural and socioeconomic backgrounds, and trauma history preferred

  • Experience charting in and working with Electronic Health Record preferred or relevant education
